Before and After Lip Fillers: Reading the Photos Properly

I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul. More patients arrive with a lip photo on their phone than with any other reference. That is fine β it tells me what you like. But there is one thing about those photos that almost nobody is told, and it changes how you should read every single one of them.
Why does the day-one photo never match the final result?
Because lips are vascular, and they swell. Mild swelling and tenderness for one to three days is normal and then settles. A photograph taken hours after the injection shows swelling plus filler β not filler. If the "after" you are comparing yourself to was shot on the day, you are comparing yourself to something that no longer exists a week later.
When you look at any before-and-after set, ask two questions: how many days after? and same lighting and angle? Lip photos are unusually easy to flatter with a light source.
What lip filler actually does
- Lip body: adds volume to upper and lower lips for a fuller shape
- Lip line: refines and defines the border for a cleaner contour
- Lip corners: subtly lifts downturned corners, which softens a tired or gloomy expression
The corners are the part patients rarely ask for and most often thank me for. A downturned corner reads as mood, not age β lifting it changes the expression more than volume does.

Who it suits β and who it doesn't
It suits thin or volume-deficient lips, blurred lip lines, and downturned corners. It is not for someone who wants a specific celebrity's mouth β lips are designed around the proportions of your whole face, and the same shape on a different face does not read the same way. I design for balance, and I will tell you when a request would look out of proportion on you.
What we do in the room, and recovery
The session is within about 30 minutes, with topical numbing ointment (local anaesthetic if needed). We use authentic HA filler in exact, measured volumes. Afterwards: mild swelling, tenderness or a small bruise for one to three days, then it settles.

What "natural" actually means in lips
Almost every patient says they want a natural result, and almost every patient means something different by it. In practice, a lip that reads as natural usually keeps three things: a visible border rather than a blurred one, a lower lip slightly fuller than the upper, and a philtrum that still shows. Lose those and the lip reads as "done" regardless of volume.
This is why I design the border and the corners as carefully as the body of the lip. Volume alone, placed without respecting those landmarks, is what produces the look patients are trying to avoid.
First time versus a top-up
A first session and a maintenance session are not the same conversation. On a first visit I place conservatively, because I would rather you return in two weeks wanting slightly more than sit with slightly too much for months. On a top-up, we are adjusting a known result and can be more precise about where the last session fell short.
If you have had lips done elsewhere, bring the details if you have them. Knowing what product was used and when changes what I can safely do today.
What patients ask me next
- "Can it be dissolved?" Hyaluronic acid can, and knowing that takes most of the fear out of a first session.
- "Why do my lips bruise more than my friend's?" Lips are vascular and vary between people. It is not a sign anything went wrong.
- "Will it stretch my lips permanently?" Placed in measured amounts and allowed to settle between sessions, no. The concern comes from repeated over-filling, which is exactly the pattern I refuse to continue.
The honest limits
It is maintained, not permanent. And the most common regret I see is not "too little" β it is the patient who kept topping up while still swollen from the last session, chasing a photograph of their own swelling. I ask patients to judge the result at two weeks, not at two days.
