Under Eye Filler Migration: Causes, Signs and How It Is Fixed

I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul and a board member of the International Petit Medical Society. Most of my clinical day is spent on fillers and injectables, and the under-eye is the area where I am most careful. The messages I receive about it are almost always the same: a photo taken in the morning and the question, "My under eye filler has moved — it's puffy below the hollow now. What happened, and can it be fixed?" The short answer: it is usually too much filler or filler placed too shallow, and hyaluronic acid can be dissolved.
What is under eye filler migration?
It is filler — or the water it holds — sitting outside the hollow it was meant to fill. The tear trough is a narrow groove under skin that is the thinnest on the face. Hyaluronic acid attracts water. When too much is placed, or it sits too close to the surface, it can spread along the loose tissue and swell, so the puffiness appears lower or wider than the original hollow.
Patients describe it as a ridge, a "sausage" under the eye, or a puffy band that is worst when they wake up. Strictly, filler rarely travels far; more often it spreads and swells where it was placed. For you the practical effect is the same, and so is the fix.
Why does it happen?
Volume, depth and anatomy — in that order. The product itself is rarely the cause.
Too much product
The under-eye needs very little. Top-ups every few months, each one small, can add up to more than the area can hold.
Too shallow
Filler close to the surface under thin skin can show as a ridge or a bluish tint. That tint is light scattering through the product, not a bruise.
The wrong under-eye for filler
Some lower lids already hold fluid easily, or have puffy fat pads. Filler adds water-holding volume to an area that is already swelling, and the bag looks bigger. In these faces I say no to filler rather than risk it.
Is it migration, or just swelling?
Give it about a week before judging. Because the under-eye skin is very thin, mild swelling or a small bruise for a few days is normal after treatment and usually settles within about a week.
What is not normal: puffiness that is still there weeks or months later, a ridge that gets worse in the morning or after salty food and alcohol, or a bluish line that does not fade. Those are worth a review. Pain that keeps getting worse, or skin that turns pale or dusky, needs a doctor the same day — that is uncommon, but it is exactly why under-eye filler should be injected by a physician.
How is migrated under eye filler fixed?
If it is hyaluronic acid, it is dissolved first. An enzyme breaks the filler down, and the area usually settles over the following days. I treat conservatively and review, because the under-eye is small and needs a light hand.
What I will not do is add more filler to disguise the excess. That is how most of these problems start. Once the old product has cleared, we decide together whether anything should go back — and if it does, less of it, placed deeper, with a blunt-tip micro-cannula.
Bring whatever you know: the product name, when it was injected, how many times it was topped up, and a photo from before. It tells me whether the filler is likely to be hyaluronic acid and how much may still be there.
How does HERA try to prevent it?
Less product, deeper placement, and a clear reason to treat. At HERA a doctor first checks what is causing the dark circle. A hollow tear trough casting a shadow responds to filler; dark circles caused mainly by pigmentation, or puffy fat pads, do not, and we tell you so.
- Blunt-tip micro-cannula to limit bruising and keep placement controlled.
- KFDA- and FDA-approved hyaluronic-acid filler, which can be dissolved if needed.
- Conservative amounts, then a review — you can always add; taking away means dissolving.

What do patients ask me next?
"Will dissolving make my under-eye worse than before?"
It returns the area towards where you started. The hollow may show again once the swelling from the old product is gone — that is the honest baseline we plan from.
"Should I just wait for it to go away?"
Hyaluronic acid does break down over time, but misplaced under-eye filler can persist longer than people expect because it keeps holding water. If it bothers you, dissolving is usually the more predictable route.
"Are there other options for under-eye hollows?"

What are the honest limits?
Filler does not remove eye bags, and it is not permanent. If the puffiness comes from fat pads rather than a hollow, filler will not fix it and can make it look worse. No injector can promise that two eyes will respond identically. What I can promise is a clear reason before treatment, measured amounts and a review afterwards.
