10THERA HIFU for a Double Chin, With a Nerve Block β Real Patient Review
Dr. Lee Jong Hoon explains Debbie's 800-line 10THERA session for her lower face and double chin β when HIFU suits a double chin, and why she chose a nerve block first.

Hello, I'm Dr. Lee Jong Hoon, the lifting specialist at Gangnam HERA and a foundation member of the Korean Academy of Facelift & Contouring. I'd like to explain Debbie's (@bydebbietan) case in my own words, because she raised two questions that almost every double-chin patient asks: can a lifting device actually help a double chin, and how much will it hurt? Debbie wrote that her double chin had "officially become one of my biggest beauty concerns," and she came to us for 10THERA HIFU lifting focused on her lower face and double chin. She was also honest with her followers that 10THERA can be painful, which is why she chose a nerve-blocking injection first. Both points deserve a straight answer.
Is a double chin a lifting problem or a fat problem?
It can be either, and very often it is a mix β which is exactly why the consultation matters more than the device. Debbie's visit started, as she describes, with skin analysis, a consultation and face mapping. What I am looking for at that stage is which component dominates under the chin. If the fullness comes mostly from loose skin and a softened support layer, so the jaw-to-neck line has lost its angle, that is a lifting problem and HIFU is a logical choice. If it comes mostly from a soft pocket of fat that you can gently pinch, that is a fat problem, and a fat-dissolving injection such as our Sok-Sok Face Injection addresses it more directly. Debbie's plan, as she shared it, was 10THERA lifting focused on the lower face and the double chin. In other words, the aim was to tighten and redefine the line under the jaw rather than to remove fat in this visit.
How does 10THERA work on the lower face and under the chin?
10THERA tightens from the deep support layer up. It is a high-intensity focused ultrasound lift that heats the SMAS β the muscular-fascial layer beneath the skin β to around 60β80Β°C, so collagen and elastin contract and then regenerate. Its 3.0mm and 4.5mm cartridges reach the dermis, the fat layer and the SMAS at the appropriate depths, and it fires two lines per shot, which makes the session roughly twice as fast as conventional single-line HIFU.
For a double chin, the mapping matters. We draw the treatment zones along the jawline and the area under the chin and decide how many lines each zone receives and at which depth. Debbie's caption mentions an 800-line treatment, and that total is distributed according to the map rather than spread evenly across the face. When the lower face and submental area firm up together, the jaw-to-neck angle becomes cleaner β that is the change a lifting approach to a double chin is aiming for.
Why did Debbie have a nerve block before 10THERA?
Because she knew she was sensitive to pain, and she wanted the session to be comfortable. Our routine is topical numbing cream, and with the faster 2-line delivery most patients find 10THERA manageable β usually brief pulses of deep heat as the energy reaches the SMAS. But pain is personal. The jawline is an area where the tissue sits close to bone, and some people feel the heat there much more strongly than others. Debbie told her followers plainly that 10THERA "can be pretty painful" and that this was why she had a nerve-blocking injection beforehand β I appreciate that honesty, because it helps the next patient plan.
A nerve block is a small injection of local anesthetic placed near the nerves that supply the area, so the treated zone is numbed more deeply than cream alone can achieve. If you know you have a low pain threshold, tell us at consultation; it is far better to plan for comfort than to cut a session short. Comfort also affects quality: a relaxed patient lets us place every line where the map says it should go.
What should you expect after 10THERA for a double chin?
Expect a gradual change, not an overnight one. Debbie wrote that 10THERA results develop gradually over the next few months, and that matches what I tell every patient. Some tightening may be visible right away, but the fuller lift builds over two to three months as new collagen forms, and results typically last around 12β18 months depending on skin condition. Downtime is minimal: mild redness or slight tenderness may appear and usually settles within a day. Results vary from person to person, and the honest place to judge a double-chin lift is a side-profile photo at around three months, compared with one taken before treatment.

What wasn't part of this plan, and what could be added later?
The treatment Debbie shared was 10THERA on its own; it did not include a fat-dissolving injection. That is a reasonable first step when looseness is a large part of the concern, because HIFU tightens tissue β it does not dissolve fat. If, after the lift has fully developed, a soft fat pocket is still the main thing you see under the chin, a fat-dissolving injection is the logical next conversation, and the two approaches work on different layers.
