Hello, I'm Dr. Kim Jong Seon, director of Gangnam HERA and the filler specialist on our team. Sophie (@sophienicolemichelle) summed up her visit in one line: "My lips finally look even." That sentence is the whole brief for a symmetry case, so I want to explain, in my own words, how we approach lips that are uneven rather than simply small. Her video shows the consultation, the numbing, the filler we used, the mirror check, and her own clips from a few hours later and one week later. I will stay with what the video and her caption show, and explain the thinking behind each step.

Why is uneven lips a different problem from thin lips?

Because the goal is balance, not size. When lips are thin, we plan volume. When lips are uneven, one side, one segment or one part of the border is usually smaller than its partner, and the task is to bring the weaker part up to meet the stronger one. If you add the same amount everywhere, the difference is still there, just on a bigger lip. So in an asymmetry case I think segment by segment: the left and right halves of the upper lip, the left and right halves of the lower lip, the border, and the corners. Our clinic's own comment on her post put it simply: "So glad to see such a balanced result." Balanced is exactly the word I use in planning.

What happened in the consultation?

In her video you can see Sophie at the consultation desk pointing to the corners of her mouth and her lips while we talk, and the facial checklist we use to mark areas on a face diagram. That conversation matters more than the injection. I ask the patient to show me what bothers her in the mirror, at rest and when she talks, because many asymmetries only appear in movement. Then I look at the border line on each side, the fullness of each half, and the corners. Lip filler at HERA is designed around the proportions of the whole face, so I also check how the lips sit relative to the nose and chin. Only when the patient and I agree on which side needs what do we move to the treatment room.

Which filler did we use, and why does that matter?

Her video shows the box and syringe clearly: Neuramis Classic, a cross-linked hyaluronic-acid filler with lidocaine, in a 1.0 mL syringe, opened in front of her. I like patients to see the product, because genuine product in a measured amount is part of what makes a lip result safe and predictable. A hyaluronic-acid filler also has an important safety margin: if a correction is ever needed, the result can be adjusted or dissolved. In an asymmetry case, what matters is that the corrected side looks, feels and moves like the other side, so the amount and placement are planned for blending, not for lift.

How was the injection done, and did it hurt?

We applied topical numbing cream to the lips first; you can see the thick white layer in her clip. Local anesthesia can be added if needed, and the filler she received also contains lidocaine. Once the area was numb, filler went in small, measured amounts, with more placed on the segments that were behind. Between passes, the patient holds a mirror and we check both sides together; that is the moment in her video where she is looking closely at her lips. Lip filler is usually finished within about 30 minutes. Her video also shows a Newlux vial, a Korean botulinum toxin, and a few small injections between the brows during the same visit. That was a separate, minor part of the appointment; the lip balance was the main work.

What do "a few hours later" and "one week later" really show?

Sophie labeled two of her own clips, and they show the normal course well. Lips are very vascular, so mild swelling, tenderness or a small bruise for one to three days is common, and lips often look fuller at first than they will finally be. Her "a few hours later" clip is exactly that early phase. The "one week later" clip is closer to the real result, because the final shape is most reliably judged after about a week. That is also why I never correct a lip asymmetry on the first day: a still-swollen lip can mislead both doctor and patient. If a small touch-up is needed, it is planned after the swelling has gone. Results vary from person to person, and swelling can be uneven for the first day or two even when the placement was symmetrical.

What did we not do, and what could come next?

We did not chase a bigger lip. Sophie's caption is about evenness, and once the weaker side matches the stronger one, adding more only changes the size, not the balance. We also did not try to correct every small difference on the first visit; lips are not perfectly symmetrical in anyone, and over-correcting one side can create a new imbalance. If a patient later wants the corners lifted or the border sharpened, that can be planned separately with fresh photos. As for longevity, domestic fillers typically last 6 to 12 months and imported fillers 12 to 18 months, though lips tend to metabolize filler a little faster than some other areas.
