저는 강남 헤라클리닉 성형외과 전문의 김종선입니다. 국제쁘띠의학회 이사로도 활동합니다. "전후사진 볼 수 있나요?"는 저희가 가장 많이 받는 메시지입니다. 당연한 질문이고, 동시에 가장 많이 오해되는 자료이기도 합니다. 그 사진이 실제로 무엇을 말해 주는지 설명드리겠습니다.

보톡스 전후 변화는 언제 나타나나요?

3~7일에 움직임이 부드러워지기 시작해 2주경 정점에 이릅니다. 시술 당일 찍은 극적인 "after"를 보여준다면 그건 보톡스가 아니라 부기입니다. 전후사진을 볼 때 가장 먼저 확인하실 것은 시술 후 며칠에 찍었는가입니다.

좋은 전후사진이 실제로 보여주는 것

  • 같은 각도, 같은 조명, 같은 표정. 평평한 조명에서 부드러워 보이는 이마는 결과가 아니라 사진입니다.
  • 정지가 아니라 움직임. 보톡스는 얼굴이 움직이는 방식을 바꿉니다. 무표정 정지 사진은 정작 치료 대상을 가립니다.

맞는 분과 맞지 않는 분

보톡스는 움직임으로 생기는 주름(이마·미간·눈가)과 교근·종아리 같은 근육 볼륨에 작용합니다. 꺼진 볼륨을 채우지 못하고 처진 피부를 끌어올리지 못합니다. 사진에 보이는 것이 움직임 주름이 아니라 꺼짐이나 늘어짐이라면, 필러나 리프팅 쪽으로 안내드리고 그렇게 말씀드립니다.

진료실에서 하는 일

헤라에서 보톡스는 전문의가 직접 주사합니다. 어떤 제품을, 몇 유닛, 어느 포인트에 넣을지는 얼굴이 움직이는 것을 본 뒤 내리는 임상적 판단이지 고정 메뉴가 아닙니다. 개봉·시리얼·기록 관리 프로토콜을 따르므로 무엇이 쓰였는지 확인하실 수 있습니다.

같은 유닛인데 두 사람이 다르게 보이는 이유

유닛은 환자분들이 생각하시는 방식의 "용량"이 아닙니다. 같은 수를 두 얼굴에 써도 다른 사진이 나옵니다. 근육량, 근력, 피부 두께, 주름이 이미 얼마나 새겨졌는지가 모두 다르기 때문입니다. 강한 미간 근육은 약한 근육보다 더 많은 양이 필요하고, 15년간 접혀온 주름에는 보톡스가 닿지 않는 피부 성분이 있습니다.

그래서 얼굴이 움직이는 것을 보기도 전에 유닛을 제시하는 곳을 경계합니다. 숫자는 진단을 따라오는 것이지 그 반대가 아닙니다.

정적 주름과 동적 주름

거울에서 무표정인 얼굴을 보십시오. 이제 찡그려 보십시오. 찡그릴 때만 생기는 주름이면 동적 주름이고 보톡스가 잘 듣습니다. 무표정일 때도 보이면 일부가 피부에 새겨진 것이라 보톡스는 완화하되 지우지 못합니다. 남은 부분은 깊이에 따라 리서페이싱·부스터·필러가 필요합니다.

이 구분을 시술 전에 이해하신 분은 2주 뒤 사진에 실망하지 않습니다. 이해하지 못한 분이 애초에 근육 문제가 아니었던 주름을 쫓아 유닛을 더 요구하십니다.

  • "얼굴이 굳어 보이지 않을까요?" 필요한 근육만 다루고 아닌 근육은 두면 그렇지 않습니다. 굳은 얼굴은 보톡스가 아니라 과다 시술의 결과입니다.

솔직한 한계

효과는 일시적이며 끝내는 것이 아니라 유지하는 것입니다. 변화의 정도는 개인차가 큽니다. 같은 유닛을 두 사람에게 써도 같은 사진이 나오지 않습니다. 부자연스러운 결과의 가장 흔한 원인은 제품이 아니라 과다 시술입니다. 몇 달을 안고 살아야 할 굳은 얼굴을 만드느니, 덜 하고 다시 오시게 하는 편을 택합니다.

  • Q3Why do before-and-after photos look so different between clinics?
    Angle, lighting and expression change a photograph more than most treatments do. Look for the same angle, the same lighting, the whole face in frame, and an interval stated in days.
  • Q4Will botox fix sagging skin?
    No. Botox works on movement lines and muscle bulk. It does not fill lost volume or lift lax skin — those need filler or lifting, and I will say so if that is what your face actually shows.
