서울에서 말하는 비수술 지방흡입(korean non surgical lipo)은 대개 지방분해 주사입니다. 작은 지방 부위 한 곳에 용액을 주입하면, 이후 몇 주에 걸쳐 몸이 치료된 지방을 처리합니다. 캐뉼라도, 절개도, 전신마취도 없기 때문에 지방흡입이 한 번의 수술로 빼는 양을 뺄 수는 없습니다. ‘귀국해서 거울을 봤는데 이번 여행이 아무것도 바꾸지 못했다면?’ 하는 걱정이 마음에 걸린다면, 물어볼 만한 질문입니다. 답은 대부분 지방의 양과 그 위 피부가 얼마나 탄탄한지에 달려 있습니다.

지방분해 주사는 실제로 무엇을 없애나요?
주사한 부위의 지방세포를 분해하는 것, 그것이 전부입니다. 몸은 치료된 지방을 천천히 처리합니다. 그래서 속속 페이스 주사(Sok-Sok Face Injection)의 슬리밍 효과는 시술 당일이 아니라 2–4주에 걸쳐 나타납니다. 얼굴에서는 턱 밑, 턱선, 볼살(버칼 지방), 옆 광대처럼 정해진 부위가 대상입니다.
얼굴 1회 시술은 10–15분이 걸리고, 보통 표면 마취 크림을 바릅니다. 몇 주 간격으로 2–3회 시술을 권하는 경우가 많습니다. 한 번의 여행에서 받는 1회 시술은 시작일 뿐, 계획의 전부가 아닙니다.
주사로 할 수 없는 일도 그만큼 중요합니다. 피부를 당기지 못합니다. 복근 아래 장기 주변에 쌓인 지방에는 닿지 않으며, 피부 쪽에서 하는 어떤 치료도 마찬가지입니다. 지방흡입도 예외가 아닙니다. 체중도 바뀌지 않습니다. 미국성형외과학회(American Society of Plastic Surgeons)는 지방흡입을 체중 감량의 대안이 아닌 체형 교정 수술로 설명합니다. 주사가 다루는 지방의 양은 수술보다 훨씬 적습니다.
맞는 사람, 그리고 제가 ‘아니요’라고 말할 때
잘 맞는 분은 그 부위가 두 손가락으로 잡힙니다. 체중이 한동안 안정적이었고, 잡았다 놓으면 피부가 바로 돌아옵니다. 이런 경우에는 정해진 부위에서 조금만 줄어도 눈으로 확인되는 경우가 많습니다.
다음 경우에는 ‘아니요’ 또는 ‘아직은 아니요’라고 말씀드립니다.
- 늘어진 피부. 앞으로 숙였을 때 턱선이나 배가 접힌다면, 아래 지방을 빼면 더 탄탄해지기보다 더 꺼져 보일 수 있습니다.
- 양이 많거나 전체적으로 날씬해 보이고 싶은 경우. 체중이나 수술의 문제입니다. 주사는 부위를 치료합니다.
- 체중 변화가 예정된 경우. 앞으로 몇 달 안에 살을 많이 뺄 계획이라면 그 부위의 모양은 어차피 달라집니다.
- 임신·수유 중, 그 부위의 발진이나 감염, 복용 중인 항응고제. 예약 전에 반드시 알려 주세요.

턱선이 처졌다면 제가 이야기할 방법은 커스텀 실리프팅이고, 20–30분이 걸립니다. 실에도 한계가 있으며, 실리프팅이 코에 할 수 있는 것과 없는 것에 정리해 두었습니다. 진짜 문제가 피부결이라면 RF 마이크로니들링이 할 수 있는 것과 없는 것이 더 맞습니다. 입가의 무거움은 지방보다 볼륨 감소에서 오는 경우가 많고, 그래서 팔자주름 필러가 주사로 풀지 못하는 고민에 답이 되기도 합니다.

알아 두어야 할 위험과 부작용
흔한 것부터 말씀드립니다. 속속 페이스 주사 후 주사 부위의 가벼운 부기, 붉은기, 작은 멍은 보통 며칠 안에 가라앉습니다. 지방분해 주사는 일반적으로 잠시 그 부위가 아프거나 피부 밑이 단단하게 느껴질 수도 있습니다.
그보다 드물게 결과가 고르지 않을 수 있습니다. 한쪽이 더 반응하거나, 1회 후 변화가 기대보다 작을 수 있습니다. 제가 가장 많이 듣는 실망이 이것이고, 대개 한 번으로 여러 번의 몫을 기대한 데서 옵니다.
드문 문제가 심각한 문제입니다. 미국 식품의약국(FDA)은 승인되지 않은 지방분해 주사 후 영구 흉터, 심각한 감염, 피부 변형, 낭종, 깊고 아픈 멍울이 생겼다는 보고를 받았습니다. 일부는 제대로 면허를 갖추지 않았을 수 있는 사람이 주사했고, 일부는 온라인으로 구입해 스스로 주사한 경우였습니다. 턱선을 따라서는 아랫입술을 움직이는 신경이 표면 가까이 지납니다. 그 부위 주사 후 드물게 일시적으로 미소가 비대칭해진 경우가 있습니다.
스스로를 지키는 방법이 몇 가지 있습니다. 제품명과 한국 식품의약품안전처 승인 여부를 물어보세요. 의사에게 주사를 받으세요. 받은 시술을 귀국 후 다니는 의사에게 알리세요. FDA도 이를 권합니다. 미국 질병통제예방센터(CDC)의 의료관광 안내는 진료 기록 사본을 챙겨 귀국하라고 권합니다. 돌아간 뒤 멍울이 커지거나 붉은기가 번지고 열감이 있으면 가까운 의사를 바로 찾고 그 기록을 가져가세요. 반응은 사람마다 다르며, 해당 부위의 최종 계획은 대면 상담에서 정합니다.
상담한 의사가 직접 주사하는 이유
이 시술은 대부분이 판단입니다. 지방인지 피부인지, 부위가 어디서 끝나는지, 얼마나 깊이 주사하는지. 저는 그 판단이 처음 보는 순간부터 마지막 주사까지 한 사람 안에 머무는 편이 낫다고 생각하고, 헤라에서는 그렇게 합니다. 상담한 의사가 치료하는 의사이며, 다른 사람에게 넘기지 않습니다.

주사 후 며칠이 지나야 비행기를 탈 수 있나요?
당일에는 마취 크림을 바르고 얼굴 1회 시술에 10–15분이 걸리며, 주사 자리에 작은 자국을 남긴 채 나가게 됩니다. 3일째쯤이면 보통 며칠 안에 가라앉는 부기와 멍이 많은 분들에게서 옅어지고 있습니다. 7일째면 대부분 평소 모습으로 돌아옵니다. 하지만 슬리밍 자체는 아직 진행 중입니다. 2–4주에 걸쳐 나타나므로 결과는 서울이 아니라 집에서 판단하게 됩니다.
출국 전 주의
출국 전날로 주사를 예약하지 마세요. 서울에 있는 동안 부기가 며칠 가라앉을 수 있도록 여행 초반에 시술을 잡으세요.
회차는 몇 주 간격이라 2회차는 보통 두 번째 방문을 뜻합니다. 상담 때 나와 비슷한 부위가 보통 몇 회 필요한지 물어보세요. 그러면 결정하기 전에 한 번의 여행으로 될지 두 번이 필요할지 판단할 수 있습니다.
한국에서 비수술 복부성형이나 팔 지방흡입이 현실적인가요?
