이 글을 쓰는 이유는 ‘제거’라는 말이 실망을 부르기 때문입니다. 많은 분이 튼살이 사라질 거라 기대하고 오시지만, 현재 그런 치료는 없습니다.
튼살은 흉터입니다. 고주파 마이크로니들링 같은 시술은 대개 여러 번에 걸쳐 튼살을 더 평평하고 덜 눈에 띄게 만들 수 있고, 붉은 초기 튼살이 오래된 흰 튼살보다 반응이 좋은 편입니다.

튼살은 왜 옅어질 수는 있어도 사라지지는 않을까?
튼살은 피부가 적응할 수 있는 속도보다 빠르게 늘어나거나 줄어들 때, 피부를 지탱하는 콜라겐과 엘라스틴이 끊어지면서 생깁니다. 미국피부과학회(AAD)는 튼살을 흉터의 한 종류로 설명하며, 치료로 덜 눈에 띄게 만들 수 있다고 말합니다. 실망스러운 여행의 대부분이 이 한 가지 사실로 설명됩니다.
찢어진 천을 꿰맨 모습을 떠올려 보세요. 꿰맨 자리를 더 평평하게, 색을 더 비슷하게 만들 수는 있지만 그 자리의 짜임은 원래 짜임으로 돌아가지 않습니다. 클리닉 시술은 진피에 조절된 자극을 주어 튼살 안에 새 콜라겐이 만들어지게 합니다. 새 콜라겐은 꺼진 부분을 부드럽게 하고 결을 고르게 할 수 있습니다. 하지만 잃어버린 탄력 섬유를 다시 만들지는 못합니다.
가장 중요한 것은 튼살의 나이입니다. 초기 튼살은 피부색에 따라 붉은색, 분홍색, 보라색 또는 짙은 갈색으로 보이고, 약간 솟아 있거나 가려울 수 있습니다. 시간이 지나면 색이 옅어지고 띠가 피부 아래로 꺼집니다. 일반적으로 초기 튼살이 반응이 좋고, 오래되어 옅고 꺼진 튼살은 변화가 적습니다.

튼살을 바꾸는 방법과 바꾸지 못하는 방법은?
대부분 이미 써 보셨을 크림부터 보겠습니다. AAD에 따르면 크림은 몇 주 동안 매일 문질러 바르면 초기 튼살에 조금 도움이 될 수 있지만, 오래된 튼살에는 효과가 거의 없어 보이고, 아몬드 오일·코코아 버터·올리브 오일·비타민 E는 연구에서 효과가 없었습니다. 셀프 태닝은 잠시 가려 줄 뿐 치료는 아닙니다.
클리닉에서 피부과 의사들은 레이저, 고주파, 마이크로니들링, 화학 박피, 미세박피를 쓰고 때로 함께 씁니다. 마이크로니들링은 미세한 구멍을 내어 콜라겐 회복을 시작하게 합니다. AAD의 마이크로니들링 안내에 따르면 고주파 마이크로니들링은 바늘을 통해 열까지 전달해 콜라겐 생성을 더 높일 수 있습니다. 같은 흉터 재구성 원리는 제 여드름 흉터 치료 가이드에도 담겨 있습니다.

예를 들어 아시아 어딘가에서 영어를 가르치는 27세이고, 열다섯 살 무렵 키가 급히 자라며 생긴 은빛 튼살이 엉덩이 옆에 있고, 서울에 5일 머문다고 해 봅시다. 저는 그렇게 오래된 튼살은 대개 변화가 완만하고, 시술 간격이 몇 주씩 필요하니 이번 여행에서는 한 번의 시술이지 한 코스가 아니라고 말씀드릴 것입니다. 다음 달 튼살이 안 보이는 해변 휴가가 목표라면 그 돈을 휴가에 쓰시는 편이 낫습니다. 내 피부가 어떻게 반응하는지 보고 다시 올 수도 있다면, 한 번의 시술로 그것을 알 수 있습니다.
대안도 실제로 있습니다. 비행기 없이 한 코스를 진행할 수 있는 현지 피부과, 초기 튼살이라면 주치의와 상의할 수 있는 처방 외용제, 혹은 오래된 튼살은 그대로 두는 것입니다.
알아 두어야 할 위험과 부작용
AAD에 따르면 마이크로니들링 후 붉어짐, 부기, 햇볕에 탄 듯한 느낌은 흔하며 대개 며칠에서 일주일 사이에 가라앉습니다. 일시적으로 색이 어두워지는 경우도 있습니다. 갈색이나 어두운 피부에서는 염증이 염증 후 과색소침착을 남길 수 있고, 이 짙은 반점은 옅어지는 데 몇 달이 걸리기도 합니다. 감염이나 새로운 흉터는 드물지만 생길 수 있습니다.
일반적인 의학 기준으로 다음은 시술을 건너뛰거나 미뤄야 할 이유입니다.
- 상처가 느리게 낫는 경우, 켈로이드 병력, 면역 저하, 해당 부위의 피부암, 해당 부위의 방사선 치료 경험
- 해당 부위의 햇볕 화상, 감염, 통증 있는 뾰루지, 또는 최근 이소트레티노인 복용
- 임신 또는 수유 중이라면 먼저 주치의와 상의
- 체중 변화·성장·임신 없이 갑자기 생긴 튼살: 장기간의 코르티코스테로이드 사용이나 쿠싱병 같은 질환이 원인일 수 있으니 먼저 현지 의사에게 원인을 확인

한국 튼살 치료 후기는 무엇을 맞게 말할까?
한국 튼살 제거에 관해 온라인에 쓰인 글은 대부분 세 가지 주장으로 모입니다. “튼살이 지워졌다”는 흉터 조직이 움직이는 방식과 다릅니다. 결과 색이 바뀌는 것이고, 조명이나 사진 각도가 그 차이를 과장할 수 있습니다. “붉은 튼살이 흰 튼살보다 더 좋아졌다”는 일반적인 피부과 지식과 맞습니다. “일주일 동안은 더 나빠 보였다”도 사실입니다. 붉어짐과 일시적인 착색은 회복 과정의 일부이기 때문입니다.
반쯤 맞는 주장도 하나 있습니다. 한국에서 한 번이면 충분하다는 것입니다. 한 번의 시술로 내 피부의 반응은 알 수 있지만, 흉터 개선은 대개 몇 주 간격의 여러 번에 걸쳐 쌓입니다. AAD가 여드름 흉터에 대해 드는 예는 2–4주 간격으로 3–5회입니다.
튼살 치료 비용은 무엇이 좌우할까?
내 경우의 비용은 몇 가지 요소로 정해집니다.
- 범위: 엉덩이 옆의 한 줄과 배·허벅지 전체는 다른 시술입니다
- 계획하는 횟수, 그리고 한 번의 여행에 하는지 여러 번에 나누는지
- 고주파 마이크로니들링에 엑소좀 부스터를 더하는지

당일. 부위가 붉고 따뜻하며 햇볕에 탄 듯합니다. AAD에 따르면 양쪽 엉덩이 옆 튼살의 마이크로니들링은 15–30분 정도 걸릴 수 있습니다. 부위에는 헐렁한 옷을 입으세요.
3일째. 많은 분이 붉은 기가 가라앉기 시작하고, 피부가 건조하거나 거칠게 느껴질 수 있습니다. 햇볕을 피하고, 피부가 가라앉을 때까지 수영장과 사우나를 피하라고 권하는 의사가 많습니다.
7일째. 대부분의 붉은 기는 대개 가라앉습니다. 새 콜라겐은 몇 주에서 몇 달에 걸쳐 만들어지니, 이번 주 거울에 보이는 모습이 결과는 아닙니다.
출국 전날은 피하세요
시술은 여행 초반에 잡아, 처음 며칠의 붉은 기가 긴 비행이 아니라 서울에 있는 동안 지나가도록 하세요.
