Forehead Filler: Who It Suits and What It Can Really Do

I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul and a board member of the International Petit Medical Society, and most of my clinical day is spent on fillers and injectables. Patients from abroad usually reach me with a side-profile photograph and the same question: "Can filler make my forehead rounder without surgery?" Here is the answer I give in consultation.
What can forehead filler actually do?
It adds soft, rounded volume to a forehead that is flat, sloped or dented, with no incision and a visible change the same day. Hyaluronic-acid filler is placed across the forehead to smooth hollows and create a gentle curve from the brow to the hairline. From the side, that curve is what makes a profile look softer and more youthful.
It does not remove wrinkles made by movement. Lines that appear when you raise your eyebrows are a muscle problem, and botox treats those — not filler. Many patients need one, some need the other, and a few need both.
Who is forehead filler for?
- A naturally flat or backward-sloping forehead
- A dent or groove across the middle of the forehead
- A forehead that has hollowed with age or weight loss
- Uneven contour, such as a slight step between the brow bone and the upper forehead
It is not the right choice if your main concern is expression lines, if you want a dramatically projected forehead, or if you have had previous forehead filler that has not been assessed. I would rather find out what is already there than add on top of it.
Forehead filler or forehead botox?
Filler changes shape; botox changes movement. If the forehead is flat or hollow at rest, filler is the tool. If the forehead only creases when you raise your brows, botox is the tool.

How is it done safely?
The forehead has important blood vessels close to the brow, so placement and depth matter more here than almost anywhere else on the face. At HERA a doctor assesses your profile first, marks the shape we are aiming for, and places the filler at the depth each zone needs, with extra care near the brows.

- Injection and shaping: the doctor molds the filler and checks symmetry with you
A session usually takes around 10 to 15 minutes of treatment time.
What happens after forehead filler?
Most people go back to their day straight away. Mild swelling, redness or a small bruise can appear and usually settles within two to three days. The forehead may feel slightly firm at first; it softens as the filler settles.
Hyaluronic-acid filler generally lasts around 6 to 12 months for domestic products and 12 to 18 months for imported ones, depending on the area and your metabolism. What patients ask me next
"Will it look too big?"
Not if the amount follows your face. A forehead that looks too full is almost always over-filled, so I plan a conservative first session and review, rather than finishing everything in one visit.
"Can it be reversed?"
Yes. Hyaluronic-acid filler can be dissolved if an adjustment is ever needed — one reason I only use it here.
"Can I do temples at the same time?"
Often, yes. A forehead and temples are one upper-face curve, and treating them together usually looks more natural. See temple filler for how that area is assessed.
How should I plan it around a trip to Seoul?
Book it early in your stay rather than the day before you fly. The forehead is best reviewed once swelling has settled, and a review visit a few days later lets me make a small adjustment if one side needs it. If you are short on time, a photo consultation before you arrive means the plan is agreed and the visit itself is quick.
Avoid heavy exercise, saunas and alcohol for the first day or two, and sleep with your head slightly raised the first night. Makeup can usually go back on the next day.
The honest limits
Forehead filler is not permanent, and it cannot reshape bone. It builds a soft curve over the structure you have. If you want a large change in projection, filler is the wrong tool and I will tell you so. What I can offer is a curve that suits your face, placed carefully, with a review rather than a single large session.
