Botox for the Forehead: What It Treats and What It Doesn't

I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul. Forehead botox looks like the simplest injectable there is, and that is precisely why it is done badly so often. The forehead has one muscle that lifts the brow, and relaxing it without thinking about that job is how a patient ends up looking tired instead of rested.
What does forehead botox actually treat?
The horizontal lines that appear when you raise your brows. Botox relaxes the frontalis so those lines soften. At HERA the same visit often covers frown lines between the brows and crow's feet at the eyes, because these muscles work together and treating one in isolation can make the others look more active by contrast.

It is not for heaviness or hooding caused by descended skin — that is laxity, and relaxing the muscle that lifts your brow will make it worse, not better. If that is what I see, I say so and point you toward lifting instead.
The brow-position question
Some people rely heavily on the frontalis to keep their brows up, often without realising it. Treat that forehead the way you would treat a strong one and the brows sit lower afterwards. This is why I watch your face move before deciding anything, and why I would rather under-dose a first visit and review you.
What we do in the room
Assessment first, then an injection of about five to ten minutes. Product placement follows the muscle I see working and the brow position I want to preserve. At HERA botox is physician-injected, with a genuine-product protocol covering opening, serial and record handling so you can verify what was used on you.
Downtime is minimal — most people return to daily activities right away. Tiny needle marks or slight redness may appear briefly; avoid rubbing the area afterwards.

"How soon will it work?"
Effects typically begin within three to seven days and generally last about three to four months. Individual results vary.
"Should I do forehead alone?"
Often not. The forehead, glabella and eye area move as a set, and treating only the forehead can leave the frown looking stronger. I will tell you when a combination makes more sense — and when it does not.
"Can I do it on a short trip?"
Yes. The procedure is short and downtime minimal, though the result settles over the following week or two rather than immediately.
Why the first session should be smaller than you expect
You can always add units at a review. You cannot remove them. With a muscle that holds your brows up, that asymmetry decides the whole approach — a conservative first dose with a two-week check costs you one extra visit; an over-dosed forehead costs you three months of looking heavy.
This is also why I am wary of a fixed unit count quoted before examination. The number should follow the muscle I watch move, not a price list.
What changes between your first and second treatment
A forehead treated for the first time is a muscle that has been working uninterrupted for years. By the second or third round, patients often need less product at longer intervals, because the muscle has spent months not over-contracting. Patients who judge cost by a single session rarely account for that.
If you have had it done elsewhere
Bring what you know — product, units, date. Knowing when the last dose was placed changes what is safe to do today, and it is the difference between building on a known baseline and guessing at one.
The honest limits
It softens movement lines; it does not fill volume loss, resurface etched lines, or lift skin. It is maintained rather than permanent. And it is not suitable during pregnancy or breastfeeding, or for those with neuromuscular disorders — disclose your medical history and medications before we plan anything.
