Forehead Botox Before and After: Reading It Honestly

Forehead botox is where most patients start, and it is also where most before-and-after photographs mislead — because the thing being treated cannot be photographed at rest.
When does forehead botox show its result?
Movement begins to soften at three to seven days and peaks near two weeks. Unlike jaw slimming, which takes weeks, the forehead responds early. If you see a dramatic "after" taken on the day of treatment, you are looking at swelling and lighting, not botox.
Why a still photo is the wrong evidence
Botox changes how a face moves. A photograph at rest shows the one state in which an untreated forehead also looks smooth. The honest comparison is the same person raising their brows before and after — and that is exactly the frame most galleries leave out.
What to check in any gallery
- Same expression — ideally brows raised in both frames
- Same lighting — flat light hides forehead lines on its own
- An interval in days, not just "after"

Static lines versus dynamic lines
Look at your forehead at rest, then raise your brows. If the line only appears when you raise, it is dynamic and botox addresses it well. If it is visible at rest too, part of it is etched into the skin — botox will soften it but not erase it, and the residual part needs resurfacing or boosters depending on depth.
Patients who understand this before treatment are never disappointed at two weeks. Patients who do not are the ones asking for more units to chase a line that was never a muscle problem.
What we do in the room
The injection takes about five to ten minutes. I watch your forehead move first — the frontalis is the muscle that lifts the brow, so treating it without regard for how much lift you rely on is how people end up with heavy brows. Dose follows what I see, not a fixed menu. Downtime is minimal; tiny needle marks or slight redness may appear briefly, and you should avoid rubbing the area.
What patients ask me next
"Will my brows drop?"
Not if the frontalis is treated with the brow position in mind. Heaviness comes from over-relaxing the only muscle that lifts the brow, which is why I dose conservatively on a first visit.
"Can I still show expression?"
That is the goal. Conservative dosing softens lines while preserving movement — a forehead that cannot move at all reads as treated, and that is not what most patients actually want.
"How long does it last?"
Generally about three to four months, varying by individual.
How to photograph your own forehead properly
If you want an honest record of your own result, take the photographs yourself before the first session. Clinic galleries are curated; your own frames are not.
- Daylight, no overhead lamp. Overhead light carves shadows into forehead lines and flatters nothing.
- Two frames each time: at rest, and with brows raised as high as they go.
- Same distance and angle. Brace your phone against something fixed.
- Repeat at two weeks, when the effect has peaked.
Why the "at rest" frame still matters
It is the one that shows etched lines — the part botox will not erase. Comparing your two rest frames tells you how much of your forehead was skin rather than muscle, which is the single most useful thing to know before deciding whether to add resurfacing later.
The honest limits
It is maintained, not permanent. Results vary because muscle strength and skin thickness differ between people, and a line folded for fifteen years has a skin component no amount of product reaches. It is also not suitable during pregnancy or breastfeeding, or with neuromuscular disorders — tell me your history before we plan.
