Thermage Before and After: What Changes, and When You See It

I'm Dr. Jong Hoon Lee, a physician at Gangnam HERA in Seoul. I co-authored LIFTINGOLOGY: Science and Clinical Practice in Aesthetic Medicine and am a foundation member of the Korean Academy of Facelift & Contouring, so lifting is most of what I do. Patients who search for Thermage before and after photos usually send me one question: "The photos online look amazing β is that what I'll really get?" My honest answer: Thermage makes skin firmer and contours cleaner, the change is real but subtle, and most of it arrives two to three months after the session.
What does a real Thermage before and after look like?
Firmer, smoother skin and a slightly sharper outline β not a different face. Thermage uses radiofrequency to heat the dermis, which contracts existing collagen and stimulates new collagen. In photos, that shows as a softer jowl edge, a cleaner line under the jaw, smoother texture on the cheeks and less crepiness around the eyes and neck.
What it does not show, if the photo is honest, is a lifted brow, a vanished jowl or a slimmer face. When a before-and-after looks that dramatic, check the lighting, the head angle and whether other treatments were done at the same time.
When do Thermage results appear?
A little tightening right away, and the fuller result over 2β3 months. Right after the session some patients notice firmer skin, partly from collagen contraction and partly from mild warmth and swelling. The part that lasts is new collagen, which takes weeks to form.
That is why I ask patients not to judge Thermage in the mirror the next morning. The fairest comparison is a photo taken before treatment and another at about three months, in the same light and at the same angle. Results typically last 12 to 18 months, varying with skin condition.
Who sees the most change β and who doesn't?
Mild to moderate laxity responds best. Thermage suits people whose skin has started to lose firmness but still has reasonable thickness.
Good candidates
- Early softening along the jawline and cheeks
- Loss of firmness together with rough texture
- Fine crepey lines around the eyes, forehead or neck
- People who want a non-surgical option with little downtime
Where Thermage is not the right tool
Heavy jowls, deep nasolabial folds or a lot of lower-face fat will not be corrected by radiofrequency alone. Heating the skin does not reposition sagging tissue. In those faces I usually talk about thread lifting, Ultherapy for the deeper SMAS layer, or fat-dissolving injections, and I say so plainly rather than sell a treatment that will disappoint.
What is Preheat Thermage at HERA?
A two-step session: Soprano Titanium laser warms the skin first, then Thermage FLX tightens it. Warming the tissue before the radiofrequency stage helps energy spread more evenly and comfortably, so the dermis is tightened more completely than with radiofrequency alone.
The session takes about 30 to 40 minutes. Topical numbing cream is applied; there are no injections or incisions. Downtime is minimal β mild redness or warmth may linger briefly and usually fades within a day. The clinical details are on our Preheat Thermage page.
How should you read Thermage before and after photos?
Compare like with like: same light, same angle, same expression, and a date. Collagen change is subtle, so small differences in lighting can look like a bigger result than the skin actually achieved.
- Look at the jawline in profile. That is where tightening shows most clearly.
- Check the timing. A photo taken the day after is showing swelling, not the result.
- Ask what else was done. Filler, threads or Botox in the same period change the picture.

What are the honest limits?
Thermage is maintenance, not a facelift. It firms skin that has started to loosen; it does not lift heavy tissue, remove fat or stop ageing. Results vary between people, and some respond more than others. What I can promise is an honest assessment of whether your skin is likely to respond before you spend anything.
