저는 강남 헤라클리닉 성형외과 전문의 김종선입니다. 국제쁘띠의학회 이사로도 활동하며, 진료 시간 대부분을 주사 시술에 씁니다. 해외에서 오는 메시지 중 점점 많아지는 유형이 있습니다. 사진 한 장과 함께 "볼 필러가 잘못된 것 같은데, 고칠 수 있나요?"라는 질문입니다. 상담에서 드리는 답을 그 순서대로 정리했습니다.

볼 필러는 왜 어색해 보이게 되나요?

거의 항상 세 가지 중 하나입니다. 양이 너무 많거나, 층이 틀렸거나, 위치가 틀린 경우입니다. 제품 자체가 원인인 경우는 드뭅니다. 히알루론산 필러는 예측 가능하게 움직입니다. 문제는 얼마나 넣었는지, 얼마나 깊이 있는지, 내 뼈와 지방에 비해 어디에 놓였는지입니다.

좋은 소식은 바로 이어집니다. 히알루론산 필러는 녹일 수 있고, 제가 보는 "잘못된" 볼 대부분은 개선할 수 있습니다. 어떻게 하는지, 손대기 전에 무엇을 확인하는지 설명드리겠습니다.

환자분들이 실제로 말하는 네 가지 문제

"얼굴이 붓고 넓어 보여요"

과다 주입입니다. 앞볼에는 한계가 있고, 그 선을 넘으면 얼굴이 올라가 보이는 대신 둥글어 보입니다. 대개 조금씩 쌓입니다. 방문할 때마다 한 번씩 보충한 양이 각각은 합리적이어도, 합치면 얼굴에 필요한 양을 훨씬 넘게 됩니다.

"필러가 너무 아래나 바깥에 있어요"

광대 위가 아니라 아래나 바깥에 볼륨이 들어가면 얼굴을 올려주지 못하고 오히려 끌어내립니다. 사진에서는 무거운 아랫볼과 평평해진 미소 라인으로 보입니다.

"덩어리나 턱이 보여요"

피부에 너무 가깝게, 또는 한 곳에 많이 넣으면 만져지거나 보일 수 있습니다. 특히 웃을 때 그렇습니다. 처음 며칠의 가벼운 울퉁불퉁함은 붓기이고, 2주가 지나도 단단한 덩어리가 남아 있다면 확인이 필요합니다.

"웃는 모양이 달라졌어요"

볼은 말하고 웃을 때마다 움직입니다. 중안면의 움직이는 부위에 볼륨이 들어가면 표정이 뻣뻣하거나 뭉쳐 보일 수 있습니다. 위치의 문제이지, 무언가 손상된 신호는 아닙니다.

붓기인가요, 정말 잘못된 건가요?

모양은 2주를 지켜본 뒤 판단하세요. 가벼운 붓기, 붉음, 작은 멍은 정상이며 보통 2~3일 안에 가라앉습니다. 최종 윤곽은 붓기가 모두 빠진 뒤에 보입니다.

예외가 하나 있습니다. 통증이 줄지 않고 오히려 심해지거나, 피부가 창백해지거나 거뭇하게 얼룩지거나, 시야에 변화가 생기면 미용 문제가 아닙니다. 당일에 의사를 만나야 합니다. 흔하지 않지만, 볼 필러는 즉시 대처할 수 있는 의사가 직접 시술해야 하는 이유가 바로 이것입니다.

잘못된 볼 필러, 고칠 수 있나요?

대부분은 가능합니다. 히알루론산 필러는 녹일 수 있기 때문입니다. 효소가 필러를 분해하고, 이후 며칠에 걸쳐 볼이 원래 상태 쪽으로 돌아갑니다. 작은 덩어리는 세심한 마사지나 아주 작은 국소 조정으로 충분할 때도 있습니다.

제가 하지 않는 것은, 필러가 많아서 생긴 문제를 필러를 더 넣어 가리는 일입니다. 가장 자주 거절하는 요청입니다. 볼이 무거워 보인다면 답은 제자리에 더 적은 양이지, 균형을 맞추려 다른 곳에 더 넣는 것이 아닙니다.

알고 계신 것을 모두 가져오세요. 제품명, 양, 날짜, 시술 전 사진입니다. 그래야 히알루론산 필러(녹일 수 있음)일 가능성과 얼마나 남아 있는지를 판단할 수 있습니다.

처음부터 피하는 방법

메뉴가 아니라 얼굴을 기준으로 계획합니다. 헤라에서는 의사가 먼저 볼륨이 실제로 어디서 빠졌는지 — 앞볼인지, 광대 앞인지, 그 아래 꺼진 부위인지 — 평가하고 주입 전에 포인트를 표시합니다. 포인트마다 필요한 깊이에 넣고, 끝나기 전에 좌우 균형을 함께 확인합니다.

  • 첫 방문은 적게. 재진에서 더할 수는 있어도, 빼기는 쉽지 않습니다.
  • 볼륨은 구조를 따른다. 필러는 광대를 받쳐 줄 뿐, 새 광대를 만들지 않습니다.
  • 처음부터 끝까지 한 의사. 계획을 세운 의사가 직접 주입합니다.

헤라의 필러는 히알루론산이며 국산과 수입 제품이 있습니다. 국산은 보통 6~12개월, 수입은 12~18개월 유지되며 부위와 대사에 따라 다릅니다. "오래 가게" 하려고 양을 늘리는 것이 잘못된 선택인 이유이기도 합니다.

환자분들이 다음으로 묻는 것

"녹이면 전보다 더 나빠지지 않나요?"

시술 전 상태 쪽으로 돌아갑니다. 처음 필러를 맞기 전에 실제 볼륨 손실이 있었다면 그 손실은 다시 보입니다. 그래서 부위가 안정된 뒤 보수적으로 다시 채우는 경우가 많습니다.

"그냥 빠질 때까지 기다리면 안 되나요?"

가벼운 과다이고 괜찮으시다면 가능합니다. 다만 볼 필러는 수개월 유지될 수 있어, 매일 신경 쓰인다면 녹이는 편이 더 빠르고 편합니다.

"실리프팅이나 리프팅으로 고칠 수는 없나요?"

진짜 문제가 볼륨이 아니라 처짐이어서 애초에 필러가 맞지 않았던 경우가 있습니다. 그렇다면 실리프팅이나 초음파 리프팅이 원인을 다룹니다. 주사기를 하나 더 드는 것보다 그렇게 말씀드리는 편을 택합니다.

솔직한 한계

필러는 영구적이지 않고, 리프팅도 아닙니다. 빠진 곳에 볼륨을 되돌릴 뿐, 처진 볼을 혼자 끌어올리거나 얼굴을 작게 만들지 못합니다. 결과와 유지 기간은 사람마다 다릅니다. 제가 약속드릴 수 있는 것은 얼굴을 기준으로 한 계획, 보수적인 첫 시술, 그리고 한 번에 많이 하기보다 재진입니다.

  • Q1What makes cheek fillers look bad?
    Usually too much volume, filler placed in the wrong layer, or filler at the wrong point on the cheek. The product itself is rarely the cause — how much was used, how deep it sits and where it was placed are.
  • Q2Can bad cheek filler be fixed?
    In most cases, yes. Hyaluronic-acid filler can be dissolved with an enzyme, and small lumps sometimes respond to massage or a small targeted adjustment. Adding more filler to hide an overfilled cheek is not the answer.
  • Q3How long should I wait before judging my cheek filler?
    About two weeks. Mild swelling, redness or a small bruise usually settles within two to three days, and the final contour shows once all swelling has gone.
  • Q4When is a problem after cheek filler urgent?
    Pain that worsens instead of easing, skin turning pale, dusky or mottled, or any change in vision. These are uncommon but need a doctor the same day.
  • Q5How long does cheek filler last?
    At HERA, domestic hyaluronic-acid fillers generally last around 6–12 months and imported ones 12–18 months. It varies with the area and your metabolism, and results vary from person to person.
