저는 강남 헤라클리닉 성형외과 전문의 김종선입니다. 국제쁘띠의학회 이사로도 활동하고 있습니다. 해외 환자분들이 내원 전 메시지로 가장 많이 묻는 부위가 코이고, 질문은 거의 같은 형태입니다. "수술 없이 코를 바꿀 수 있나요?" 상담에서 드리는 답을 그대로 적었습니다.

코 필러로 수술 없이 코가 정말 바뀌나요?

됩니다. 다만 범위가 있습니다. 코 필러는 절개 없이 콧대 높이를 만들고 코끝 윤곽을 정리하며, 변화는 당일 보입니다. 헤라에서는 히알루론산 필러에 KFDA 승인 민트실을 함께 쓰는 경우가 많습니다. 필러만으로는 유지하기 어려운 코끝의 매끈한 리프팅 라인을 실이 잡아 주기 때문입니다.

반대로 코를 작게 만들지는 못합니다. 필러는 볼륨을 더하는 것이지 뼈·연골·폭을 줄이지 못합니다. 콧볼이 넓거나 코끝이 큰 것이 고민이라면, 나오지 않을 결과를 파는 대신 상담에서 "필러는 맞는 수단이 아닙니다"라고 말씀드립니다.

맞는 분과 맞지 않는 분

  • 콧대가 낮거나 평평한 경우
  • 코끝이 둥글고 윤곽이 불분명해 또렷하게 하고 싶은 경우
  • 콧대의 작은 굴곡이나 얕은 옆선
  • 수술을 고민하기 전에 미리 변화를 시험해 보고 싶은 경우

축소를 원하시거나, 재수술을 여러 번 해 반흔이 많거나, 수술 수준의 변화를 기대하신다면 맞지 않습니다. 먼 길 오셔서 실망하시는 것보다 미리 말씀드리는 편을 택합니다.

진료실에서 실제로 하는 일

시술은 약 30분입니다. 먼저 마취 연고를 바르고, 필요하면 국소마취를 합니다. 미간-콧대 연결부, 콧대, 코끝을 얼굴 전체와 견주어 설계합니다. 코는 옆선과 비율로 판단하는 부위이지 단독으로 보는 부위가 아닙니다. 그다음 실과 필러를 계량된 양으로 넣습니다.

환자분들이 꼭 아셨으면 하는 것이 하나 있습니다. 되돌릴 수 있습니다. HA 필러는 녹일 수 있습니다. 수술을 망설이는 분께 권할 수 있는 이유가 바로 이것입니다.

회복과 경과

대부분 바로 일상으로 돌아가십니다. 가벼운 발적·부기·코끝 눌림감은 흔하고 며칠 안에 가라앉습니다. 며칠간 무거운 안경을 콧대에 얹거나 압력을 주지 않도록 부탁드리고, 여행 일정을 알려 주시면 그에 맞춰 계획합니다.

결과는 즉시 보이며, 부기가 빠지는 1~2주에 걸쳐 자리를 잡습니다.

필러냐 실이냐, 아니면 둘 다냐

두 가지를 경쟁 관계로 생각하고 오시는 분이 많습니다. 코에서는 대개 짝입니다. 필러는 콧대를 세우고, 실은 코끝에 투사와 깔끔한 라인을 줍니다. 코끝은 움직이고 장력을 받는 부위라 필러만으로는 몇 주 안에 그 라인을 잃기 쉽습니다. 어떤 조합이 필요한지는 옆선의 어디가 부족한지에 달렸습니다. 콧대만 낮고 코끝이 괜찮으면 실 작업은 거의 필요 없고, 콧대는 충분한데 코끝이 뭉툭하면 대부분 실의 문제입니다.

이 판단은 미리 보내주신 사진이 아니라 거울 앞에서 함께 내립니다. 사진은 옆선을 평평하게 만드는데, 우리가 다루는 것이 바로 그 차원이기 때문입니다.

나중에 수술하고 싶어지면요?

정말 자주 나오는 질문이고 분명히 답할 가치가 있습니다. 필러를 했다고 코성형의 문이 닫히지 않습니다. 중요한 건 시점과 고지입니다. 무엇을 언제 넣었는지 집도의에게 알리시고, 히알루론산이라면 미리 녹일 수 있습니다. 그러면 집도의가 증강된 상태가 아니라 본래 해부학 위에서 수술합니다.

망설이는 분께 비수술 경로의 진짜 장점이 이것입니다. 변화의 한 버전을 보고, 겪어보고, 그러면서도 모든 선택지를 열어둘 수 있습니다.

환자분들이 다음으로 묻는 것

  • "티 나지 않을까요?" 양이 맞으면 나지 않습니다. 코 필러가 잘못되는 건 콧대가 감당 못 할 높이를 쫓을 때입니다. 그러면 올라가는 게 아니라 넓어집니다.
  • "전에 맞은 필러도 교정되나요?" 대개 됩니다. 녹이고 다시 시작하는 건 정상적인 과정이고, 제가 넣지 않은 기반 위에 쌓느니 초기화하는 편을 택합니다.
  • "안경은 언제부터요?" 며칠 두시고, 시술 직후 콧대에 무거운 안경테가 얹히지 않게 하십시오.

코 필러는 영구적이지 않습니다. 끝내는 시술이 아니라 유지하는 시술입니다. 또한 코는 혈관 해부학 때문에 얼굴 어느 부위보다 주입 술기가 중요한 곳입니다. 그래서 의사의 손이어야 하고, 불필요한 위험이 있다고 판단되면 저는 주사를 거절합니다.

  • Q3Can nose filler make my nose smaller?
    No. Filler adds volume; it cannot remove bone, cartilage or width. If reduction is your goal, filler is the wrong tool and I will say so at the consultation.
