저는 강남 헤라클리닉 성형외과 전문의 김종선입니다. 국제쁘띠의학회 이사로도 활동하며, 진료 시간 대부분을 필러와 주사 시술에 씁니다. 그중 가장 조심하는 부위가 눈밑입니다. 눈밑에 관해 받는 메시지는 거의 같습니다. 아침에 찍은 사진 한 장과 함께 "눈밑 필러가 움직였어요. 꺼진 곳보다 아래가 부어 있어요. 왜 이런 건가요, 고칠 수 있나요?" 짧게 답하면, 대부분 양이 많거나 너무 얕게 들어간 경우이고, 히알루론산이라면 녹일 수 있습니다.

눈밑 필러 이동이란 무엇인가요?

필러, 또는 필러가 머금은 수분이 원래 채우려던 꺼짐 밖에 자리 잡은 상태입니다. 눈물고랑은 얼굴에서 가장 얇은 피부 아래의 좁은 홈입니다. 히알루론산은 물을 끌어당깁니다. 양이 많거나 표면 가까이 있으면 느슨한 조직을 따라 퍼지고 부어서, 원래 꺼진 자리보다 아래나 옆이 볼록해 보입니다.

환자분들은 능선, 눈밑 "소시지", 아침에 가장 심한 볼록한 띠라고 표현합니다. 엄밀히 말하면 필러가 멀리 이동하는 경우는 드물고, 대개 넣은 자리에서 퍼지고 붓습니다. 하지만 환자분 입장에서 결과는 같고, 교정 방법도 같습니다.

왜 생기나요?

양, 깊이, 해부학적 구조 — 이 순서입니다. 제품 자체가 원인인 경우는 드뭅니다.

양이 너무 많을 때

눈밑은 아주 적은 양만 필요합니다. 몇 달마다 조금씩 보충한 것이 쌓이면 이 부위가 감당할 수 있는 양을 넘습니다.

너무 얕을 때

얇은 피부 바로 아래 필러는 능선이나 푸르스름한 빛으로 보일 수 있습니다. 이 푸른빛은 멍이 아니라 필러를 통과하며 산란된 빛입니다.

필러가 맞지 않는 눈밑

원래 부종이 잘 생기거나 지방 주머니가 볼록한 아래눈꺼풀이 있습니다. 이미 붓기 쉬운 곳에 물을 머금는 볼륨을 더하면 주머니가 더 커 보입니다. 이런 얼굴에는 위험을 감수하기보다 필러를 하지 않는다고 말씀드립니다.

이동인가요, 그냥 붓기인가요?

일주일 정도 지켜본 뒤 판단하세요. 눈밑 피부는 매우 얇아서 시술 후 며칠간 가벼운 붓기나 작은 멍은 정상이며, 보통 일주일 안팎에 가라앉습니다.

정상이 아닌 것은 몇 주, 몇 달이 지나도 남는 볼록함, 아침이나 짠 음식·술 뒤에 더 심해지는 능선, 사라지지 않는 푸른 선입니다. 이런 경우는 확인이 필요합니다. 통증이 계속 심해지거나 피부가 창백해지거나 거뭇해지면 당일 의사를 만나야 합니다. 흔하지 않지만, 눈밑 필러를 의사가 직접 해야 하는 이유가 바로 이것입니다.

이동한 눈밑 필러는 어떻게 교정하나요?

히알루론산이라면 먼저 녹입니다. 효소가 필러를 분해하고, 보통 며칠에 걸쳐 가라앉습니다. 눈밑은 작고 섬세한 부위라 보수적으로 시술한 뒤 경과를 봅니다.

하지 않는 것은 과한 필러를 가리려고 필러를 더 넣는 일입니다. 이런 문제 대부분이 거기서 시작합니다. 기존 필러가 빠진 뒤, 다시 넣을지 함께 결정합니다. 넣는다면 더 적게, 더 깊게, 끝이 뭉툭한 마이크로 캐뉼라로 넣습니다.

알고 계신 정보를 가져오세요. 제품명, 시술 시기, 몇 번 보충했는지, 시술 전 사진입니다. 히알루론산일 가능성과 남아 있는 양을 판단하는 데 도움이 됩니다.

헤라는 어떻게 예방하나요?

적은 양, 깊은 위치, 그리고 시술할 분명한 이유입니다. 헤라에서는 먼저 다크서클의 원인을 확인합니다. 꺼진 눈물고랑이 만든 그림자는 필러로 좋아지지만, 색소가 주원인인 다크서클이나 볼록한 지방 주머니는 좋아지지 않으며, 그렇다고 말씀드립니다.

  • 식약처·FDA 승인 히알루론산 필러를 써서 필요하면 녹일 수 있습니다.

솔직한 한계는 무엇인가요?

필러는 눈밑 지방 주머니를 없애지 못하고, 영구적이지도 않습니다. 볼록함이 꺼짐이 아니라 지방 주머니에서 온다면 필러로 해결되지 않고 오히려 더 도드라질 수 있습니다. 두 눈이 똑같이 반응한다고 약속할 수 있는 의사는 없습니다. 제가 약속드릴 수 있는 것은 시술 전 분명한 이유, 정해진 양, 그리고 시술 후 재진입니다. 필러가 처음이시라면 처음 받는 한국 필러 가이드부터 보시길 권합니다.

    Usually too much product, filler placed too close to the surface, or repeated top-ups in thin skin that holds water easily. What patients call migration is often filler spreading or swelling beyond the hollow it was meant to fill.
  • Q2How do I know if my under eye filler has migrated?
    A soft puffy ridge or 'sausage' below the eye that is worse in the morning, a bluish tint, or puffiness that sits lower than the original hollow are the typical signs. Normal swelling after treatment settles within about a week.
  • Q3Can migrated under eye filler be dissolved?
    Yes, if it is hyaluronic-acid filler. An enzyme breaks it down, and the area is allowed to settle before deciding whether anything should be placed again.
  • Q4Can I get under eye filler again after it migrated?
    Sometimes, but only after the old product is cleared and only if the anatomy suits filler. When less is placed, deeper, with a blunt cannula, the result is usually more stable.
  • Q5How long does under eye filler last?
