저는 강남 헤라클리닉 성형외과 전문의 김종선입니다. 국제쁘띠의학회 이사로도 활동하며, 진료 시간 대부분을 필러와 주사 시술에 씁니다. 한국 필러 전후 사진을 찾아보신 분들은 캡처 한 장과 함께 이렇게 물으시곤 합니다. "시술하면 정말 이렇게 되나요?" 솔직한 답은 이렇습니다. 좋은 필러 결과는 쉬어 보이고 균형이 더 잡힌 본인의 얼굴입니다. 꺼진 곳은 부드럽게, 윤곽은 깔끔하게, 비율은 고르게 됩니다. 다른 사람처럼 보여서는 안 되며, 전후 사진이 그렇게 보인다면 사진을 어떻게 찍었는지부터 살펴보겠습니다.
실제 한국 필러 전후는 어떻게 보이나요?
볼륨이 더 잘 받쳐진 같은 얼굴입니다. 꺼진 곳은 부드러워지고, 윤곽은 정돈되고, 비율은 고르게 됩니다. 히알루론산 필러는 빠졌거나 원래 부족했던 곳에 볼륨을 더하고, 주름을 아래에서 받쳐 매끄럽게 합니다. 사진에서는 더 밝은 눈밑, 볼에서 턱으로 이어지는 매끄러운 전환, 옆에서 본 둥근 이마, 더 도톰하고 또렷한 입술로 나타납니다.
제가 보기에 한국식 접근이 더하는 것은 디자인의 절제입니다. 목표는 과하게 채운 모습이 아니라 얼굴에 맞는 입체적 균형입니다. 그래서 정직한 "후" 사진은 기대보다 조용한 경우가 많고, 변화는 정면 셀카보다 측면과 움직임에서 더 분명합니다.
결과는 언제 판단할 수 있나요?
변화는 바로 보이지만, 모양은 붓기가 가라앉은 뒤 판단하세요. 보통 2~3일 뒤, 입술과 눈밑은 일주일쯤 뒤입니다. 시술 직후 가벼운 붓기, 홍조, 작은 멍은 흔합니다. 입술은 혈관이 많아 처음엔 더 커 보일 수 있고, 눈밑은 피부가 아주 얇아 며칠간 작은 멍이 보일 수 있습니다.
그래서 당일이나 다음 날 아침에 찍은 사진은 공정한 "후"가 아닙니다. 서울을 방문하신다면 출국 전에 재진할 수 있도록 일정 초반에 시술을 잡으세요. 직접 사진도 남겨 두세요. 같은 방, 같은 조명, 같은 각도로 시술 전과 일주일쯤 뒤에 찍으시면 됩니다.
부위별로 무엇이 가장 분명하게 달라지나요?
변화가 가장 잘 보이는 부위는 분명한 꺼짐이나 정돈할 분명한 윤곽이 있는 곳입니다. 히알루론산 필러로 흔히 시술하는 부위와, 각 부위에서 현실적인 "후"가 보여 주는 것입니다.

필러 전후 사진은 어떻게 정직하게 읽나요?
같은 조건끼리 비교하세요. 같은 조명, 같은 각도, 같은 표정, 그리고 두 사진 모두에 날짜가 있어야 합니다. 필러 변화는 몇 밀리미터의 윤곽인 경우가 많아서, 조명이나 고개 각도만 바뀌어도 시술보다 큰 변화처럼 보일 수 있습니다.
- 조명을 보세요. 위에서 오는 빛은 꺼짐을 깊게, 부드러운 정면광은 꺼짐을 지웁니다. "전" 사진의 빛이 더 거칠다면 비교가 과장됩니다.
- 각도를 보세요. 이마, 턱, 턱선 결과는 측면에서, 눈밑 결과는 정면에서 보입니다.
- 시점을 보세요. 당일 사진은 결과가 아니라 붓기를 보여 줍니다.
- 다른 시술을 물어보세요. 같은 기간의 보톡스, 실리프팅, 피부 시술이 사진을 바꿉니다.

"후"는 얼마나 유지되나요?
헤라 기준으로 국산 필러는 보통 6~12개월, 수입 필러는 12~18개월 정도 유지됩니다. 유지 기간은 부위(입술처럼 움직임이 많은 부위는 조금 더 빨리 흡수되는 편입니다)와 개인 대사에 따라 다릅니다. 결과는 사람마다 다릅니다.
