이 글을 쓰는 이유는 실제로는 꺼짐인데 지방 재배치 비용을 알아보는 분이 많고, 반대로 필러로는 펼 수 없는 불룩함에 필러를 예약하는 분도 많기 때문입니다.
지방 재배치는 하안검 수술이고, 비용은 주로 수술 방식, 마취, 실밥과 경과 확인을 위해 머무는 기간에 따라 정해집니다. 헤라의 시술 메뉴에는 없습니다. 제가 드리는 것은 눈밑 필러(아이 리주버네이션 필러)로, 불룩한 지방보다는 꺼짐에 맞는 시술입니다.

눈밑지방인가요, 꺼짐인가요? 어떤 방법이 맞을까요?
아래 눈꺼풀 밑에는 작은 지방 덩어리 세 개가 얇은 막에 눌려 있습니다. 나이가 들면 이 막이 대개 느슨해져 지방이 앞으로 밀려 나와 불룩해집니다. 동시에 바로 아래 볼은 얇아지는 경우가 많아 불룩함 밑에 홈, 즉 눈물고랑이 생깁니다. 피곤해 보이는 눈은 대부분 이 두 가지가 섞여 있습니다.
사람들이 찾아보는 방법들은 이 그림의 서로 다른 부분을 다룹니다.
- 지방 재배치는 수술로, 보통 아래 눈꺼풀 안쪽으로 접근합니다. 튀어나온 지방을 풀어 홈 위로 덮어 주어 불룩함과 꺼짐을 한 번에 고르게 합니다.
- 지방 이식은 허벅지나 배 등 다른 부위의 지방을 채취해 눈밑에 주입합니다. 볼륨은 더하지만 불룩함은 해결하지 않습니다.
- 눈밑 필러(아이 리주버네이션 필러)는 히알루론산을 홈에 넣어 그늘을 들어 올립니다. 소량의 볼륨을 더할 뿐 지방을 옮기지는 않습니다.

예를 들어 56세이고 밴쿠버에서 열흘 일정으로 오시는데, 올려다볼 때 눈밑이 확실히 불룩해진다고 해 봅시다. 필러는 불룩함을 더할 뿐이니, 이번 여행은 필러가 아니라 수술을 고민할 문제라고 말씀드리겠습니다. 반대로 44세이고 불룩함은 거의 없는데 오후만 되면 컨실러로도 가려지지 않는 그늘이 진다고 해 봅시다. 이런 얼굴이 눈밑 필러가 맞는 경우이고, 짧은 체류로도 가능한 경우입니다.
지방과 혼동되는 것이 두 가지 더 있습니다. 비립종은 작은 하얀 각질 알갱이로, 수술이나 필러로 치료되지 않으며 의사가 따로 제거합니다. 얇고 잔주름진 피부는 피부결의 문제로, 이때는 리쥬란 같은 스킨부스터가 상담에서 거론되기도 합니다.

한국 눈밑지방 재배치 비용은 하나의 숫자가 아닙니다. 수술 자체가 사람마다 다르기 때문입니다. 보통 비용을 움직이는 요소는 이렇습니다.
- 접근 방식. 피부 절개 없이 눈꺼풀 안쪽으로 하는 수술과, 늘어진 피부를 제거하려고 속눈썹 아래를 절개하는 수술은 규모가 다릅니다.
- 함께 하는 것. 눈꺼풀 고정, 지방 이식, 윗눈꺼풀 수술을 함께 하면 수술 시간이 늘어납니다.
- 마취. 수면 동반 국소마취와 전신마취는 필요한 시설과 인력이 다릅니다.

"눈밑 필러는 항상 이동한다." 일부는 맞습니다. 눈밑 피부는 얼굴에서 가장 얇아서 양이 너무 많거나 너무 얕게 들어가면 붓기나 푸르스름한 빛으로 보일 수 있습니다. 맞는 사람에게 소량을 깊게 넣는 것은 다른 상황입니다. 틀린 부분은 "항상"이라는 말입니다.
"지방 재배치는 평생 간다." 옮긴 지방은 대체로 놓인 자리에 머물러 변화가 여러 해 가는 경향이 있습니다. 다만 피부와 볼은 계속 나이가 들기 때문에 그 부위도 시간이 지나면 변합니다.
"한국 눈밑 밤이나 컨실러로 눈밑지방이 줄어든다." 크림은 피부에 수분을 줄 수 있고 컨실러는 그늘을 가릴 수 있지만, 어떤 바르는 제품도 지방을 옮기지는 못합니다. 밤 후기에서 눈밑이 사라졌다고 한다면 그것은 가림이거나 붓기가 빠진 것입니다.
"지방 이식은 회복이 빠르다." 지방 이식은 보통 필러보다 더 붓고, 이식한 지방이 얼마나 살아남는지가 사람마다 달라 작은 멍울이 생길 수 있습니다. 수술이므로 회복도 수술의 회복입니다.

짧은 일정이라면 회복은 어떻게 될까요?
눈밑 필러의 일반적인 경과는 다음과 같습니다.
- 당일: 약간의 붓기와 주사 부위의 작은 멍이 있을 수 있습니다. 대부분 호텔로 돌아가 조용한 저녁을 보냅니다.
- 3일째: 붓기는 대개 가라앉지만 아침에는 약간 부어 보일 수 있습니다. 작은 멍은 컨실러로 가릴 수 있습니다.
- 7일째: 대개 붓기가 대부분 빠지고 필러가 만든 모양이 보입니다.
지방 재배치 수술은 더 긴 시계로 움직입니다. 멍과 붓기가 1–2주 가는 경우가 많고, 속눈썹 아래 절개가 있으면 실밥을 뽑아야 하므로 대부분의 수술 계획은 필러 방문보다 서울에 더 오래 머물게 됩니다.
비행 전 주의
귀국 비행 전날에는 시술을 받지 마세요. 미국 질병통제예방센터(CDC)의 의료관광 안내는 여행 전에 사후 관리를 계획하라고 권하며, 수술 직후 비행은 혈전 위험을 높일 수 있다고 설명합니다.
알아 두어야 할 위험과 부작용
눈밑 필러 후 흔한 반응은 붓기, 멍, 압통, 작은 멍울이며 대개 며칠에서 2주 안에 옅어집니다. 제품이 너무 얕게 있으면 붓기나 푸르스름한 빛이 남을 수 있습니다. 미국 FDA는 드물지만 심각한 위험으로 필러가 혈관에 들어가 피부를 손상시키거나 아주 드물게 시력에 영향을 줄 수 있다고 밝힙니다. 눈꺼풀 수술에도 안구건조, 비대칭, 아래 눈꺼풀이 당겨 내려가는 것 등 고유의 위험이 있습니다.
일반적인 의학 기준으로, 다음에 해당하면 미루거나 다른 방법을 택하는 것이 좋습니다.
