I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul and a board member of the International Petit Medical Society, and most of my clinical day is spent on fillers and injectables. Patients who write to me about their chin almost always attach a side-profile photograph and ask the same thing: "Can filler give me a better chin without surgery — and will it still look like my face?" My answer is yes, if your chin is short or sits back and you want a moderate, reversible change. It is no, if the real issue is width, fat or muscle. The rest of this article is how I tell the difference.

What can chin filler actually do?

Chin filler brings a short or set-back chin forward and adds a little length, so the lower face and profile look more balanced — with no incision and a visible change the same day. Hyaluronic-acid filler is placed at the chin to build shape over the structure you already have. In profile, the nose, lips and chin read as one line. When the chin sits behind that line, the nose can look larger and the lower face less defined than it really is, and a small amount of forward projection often changes the whole profile more than people expect.

Filler can also smooth a slightly flat or uneven chin surface. What it cannot do is make a chin narrower, remove fullness underneath it, or relax a jaw muscle. Those are different problems, and I will come back to them below.

Who is chin filler for — and who isn't?

It suits a chin that is short, retruded or slightly flat, in someone who wants a moderate change they can adjust later. The patients I see do well with filler usually fall into one of these groups:

  • A chin that sits behind the line of the lips when seen from the side
  • A short lower face where the chin looks small for the rest of the features
  • A flat or slightly uneven chin surface
  • Someone who wants to see a new chin shape before thinking about anything surgical

It is not the right tool in four situations. If the fullness is under the chin, that is usually fat, and our Sok-Sok Face injection targets the double chin and jawline instead. If the lower face looks wide and square, that is often the masseter muscle, which botox treats — our guide to botox for the jawline explains how. If you want a very large change in projection, filler is the wrong tool and I will say so. And if you have had chin filler before that has not been assessed, I want to find out what is already there before adding anything.

What does a "Korean-style" chin really mean?

It means a chin designed in proportion to the nose, lips and jawline, not a chin made as long or as pointed as possible. Many patients show me photographs of a slim, tapered lower face. The part of that look filler can contribute is the chin tip and the way it flows into the jawline; the width of the jaw usually comes from muscle and bone, which filler does not change.

That is why I rarely plan a chin on its own. In our C-line lifting filler work, the forehead, cheeks and jawline are treated as one continuous curve, and the chin is where that curve finishes. How is chin filler done at HERA?

A doctor assesses your profile first, designs the chin before any injection, and places the filler at the depth the chin needs, in a session of about 10 to 15 minutes. The steps are the same every time:

  • Consultation and analysis: your profile, facial proportions and what you want to change
  • Design and marking: the new chin line is drawn before a needle is picked up
  • Anaesthesia: topical ointment, or local anaesthesia where needed
  • Injection and shaping: hyaluronic-acid filler is placed in measured amounts and gently moulded
  • Symmetry check: we look at the front and side together before you leave

Filler anywhere on the face carries a small risk of affecting blood flow. The signs are pain that keeps getting worse, or skin that turns pale, dusky or mottled, or blisters. That needs a doctor the same day. It is uncommon, and it is exactly why chin filler should be injected by a physician who can act at once.

What is recovery like, and how long does it last?

Most people return to their day straight away; mild swelling, redness or a small bruise can appear and usually settles within two to three days. The chin may feel slightly firm at first and softens as the filler settles. I ask patients to avoid heavy exercise, saunas and alcohol for the first day or two.

Hyaluronic-acid filler generally lasts around 6 to 12 months for domestic products and 12 to 18 months for imported ones, depending on the area and your metabolism. What patients ask me next

These three questions come up in almost every chin consultation.

"Will it make my face look longer?"

Not if the amount follows your proportions. A chin that looks too long is almost always over-filled, so I plan a conservative first session and review, rather than finishing everything in one visit. You can always add; taking away means dissolving.

"Can it be reversed?"

Yes. Hyaluronic-acid filler can be dissolved if an adjustment is ever needed — one reason it is the filler I use in the chin.

"Can I do my chin and jawline in the same visit?"

Often, yes. Several facial areas can be balanced in one visit, and the chin usually looks more natural when it is planned with the jawline. The first-timer's guide to fillers in Korea explains how a visit runs if this is your first time.

The honest limits

Chin filler is temporary, and it cannot change bone. It builds a soft projection over the chin you have, and it wears off over months, so it is something you maintain rather than a one-time change. It will not narrow a wide chin, remove a double chin or replace surgery for someone who needs a large change. If you are planning it around a trip to Seoul, book it early in your stay rather than the day before you fly, so a review visit a few days later is possible.
