Nasolabial folds β the lines from the sides of the nose to the corners of the mouth β are one of the most common reasons patients message me before a trip, and the cost question usually comes with a hint of suspicion: "Why can't anyone just tell me what nasolabial fold filler costs?" My answer first: because the price of softening a fold depends on why it deepened. A fold caused by lost volume in the cheek above it is planned differently from a fold caused by sagging, and filling the crease alone is often the cheapest-looking option and the least effective one. Why doesn't nasolabial fold filler have one price?
Because the fold is a symptom, and the estimate follows the cause β volume loss, sagging, skin quality, or a mix of the three. Everyone has a nasolabial fold; it is part of how the face moves when you smile. It starts to look deeper at rest when the mid-face loses some of its support and the tissue sits a little lower. For cost, what matters is this: if lost volume is the main cause, filler replaces it and the plan is relatively simple. If sagging is the main cause, filler alone tends to look heavy, and the money is better spent on lifting. A single number online cannot know which of those you have, and a number that assumes one of them will be wrong for many people who read it.

Are you paying to fill the fold, or to support what sits above it?
Often both β and the support above the fold is frequently the larger and more useful part of the plan. A modest amount of filler placed to support the cheek can soften a fold more naturally than a larger amount pushed straight into the crease. How does the depth of the fold change the amount?
A fold visible only when you smile usually needs no filler; a shallow fold at rest needs a small amount; a deep, long-standing fold may need more, staged over two visits. Here is how I think about it in consultation:
- Only when smiling: that movement is normal. Filling it tends to look stiff, and I will tell you not to pay for it.
- Visible at rest, shallow: a small, well-placed amount is often enough, sometimes with light cheek support.
- Deep and long-standing: the fold has usually formed over years, and it is wiser to soften it in two steps β a conservative first session, then a review once swelling has settled β than to chase it in one visit.
- Uneven sides: one fold is often deeper than the other, and the amounts are planned per side.
Mild swelling, redness or a small bruise usually settles within two to three days. If a second step is needed, it is agreed at the review and written as its own line, so you only pay for what the review shows is useful.
Does domestic or imported filler change the estimate?
Yes β the product is a separate line, and the practical difference between the two is how long the result tends to last. At HERA, domestic hyaluronic-acid fillers typically last around 6 to 12 months and imported ones around 12 to 18 months, depending on the area and your metabolism. Both are hyaluronic acid, both are placed the same way, and both can be dissolved if an adjustment is ever needed.
The fold sits in one of the most mobile parts of the face, so I think about the product in terms of a year rather than a visit. If you come to Seoul often, a domestic product maintained more regularly may suit you; if you come once a year, a longer-lasting product may fit your calendar better. When is filler the wrong thing to pay for?
When sagging, not volume, is the main reason the fold has deepened β and saying so is part of an honest estimate. Adding volume to a descended mid-face makes it heavier, and the fold often looks no better. In that case the options I consider at HERA are different treatments with different estimates:

I would rather move the money to the tool that fixes the cause than sell you a syringe that cannot.
What does an itemised nasolabial estimate show?
Every step to do this area properly, written down before anything begins. A typical plan covers consultation and facial analysis; design and marking of the injection points; topical ointment or local anaesthesia; injection at the depth each zone needs; shaping and a symmetry check with you; and aftercare guidance. Treatment usually takes around 10 to 15 minutes, and most people go back to their day straight away.
Anything outside that core β dissolving earlier filler, cheek support, a review top-up, or another area such as the under-eyes β is listed separately so you can decide item by item. The area beside the nose has important blood vessels, so technique matters more than the product name: pain that keeps getting worse, or skin turning pale, dusky or mottled, needs a doctor the same day.
