저는 강남 헤라클리닉 성형외과 전문의 김종선입니다. 국제쁘띠의학회 이사로도 활동합니다. 사진을 보내며 "보톡스로 턱선이 갸름해질까요?"라고 물으실 때, 제가 먼저 할 일은 된다·안 된다를 답하는 것이 아니라 셋 중 어느 문제인지 가려내는 것입니다.

보톡스로 턱선이 정말 갸름해지나요?

넓이의 원인이 근육일 때만 그렇습니다. 보톡스는 발달한 교근의 부피를 몇 주에 걸쳐 줄입니다. 뼈에는 아무 작용도 하지 않고, 내려온 피부를 끌어올리지도 못합니다. 이 셋은 사진에서 거의 똑같이 보이는 별개의 문제입니다.

어느 쪽인지 구분하는 법

근육

어금니를 물고 귓불 바로 앞, 턱 각 부위를 만져 보십시오. 악물 때 단단하게 불룩해졌다가 힘을 빼면 부드러워지면 교근입니다. 보톡스가 다루는 것이 이것입니다.

뼈

악물든 안 물든 넓이가 그대로이고 가장자리가 단단하게 고정되어 있으면 하악골입니다. 어떤 주사도 뼈를 바꾸지 못합니다.

처짐

넓다기보다 윤곽이 흐릿하다면 — 심술보, 턱과 목 사이 선이 뭉개진 상태 — 탄력의 문제입니다. 이완이 아니라 리프팅이 필요합니다. 여기에 보톡스를 더하면 아래 얼굴이 오히려 더 비어 보일 수 있습니다.

진료실에서 하는 일

사진이 아니라 촉진으로 판단합니다. 사진은 그 불룩함이 근육인지 뼈인지 알려주지 않기 때문입니다. 근육이라면 주사는 5~10분입니다. 악물어 경계를 확인하고, 그 안쪽 깊은 층에, 만져지는 근육을 따라 용량을 정해 넣습니다.

근육의 변화는 3~7일에 시작되지만, 원하시는 갸름함은 쓰이지 않는 부피가 줄면서 몇 주에 걸쳐 나타납니다. 효과는 대개 3~4개월 지속됩니다.

환자분들이 다음으로 묻는 것

"리프팅과 같이 해도 되나요?"

대개 됩니다. 오히려 그래야 할 때도 있습니다. 근육 부피와 초기 처짐이 함께 있는데 한쪽만 다루면 절반만 만족하십니다. 다운타임이 여행 일정과 맞도록 순서를 조정합니다.

"근육과 뼈가 둘 다면요?"

보톡스는 그중 일부를 줍니다. 대략 어느 부분인지 말씀드립니다. 시작 전에 그걸 아는 것이 만족한 환자와 실망한 환자의 차이입니다.

"짧은 일정에도 괜찮을까요?"

괜찮습니다. 다운타임이 적어 대부분 바로 일상으로 돌아갑니다. 다만 그 여행 중에 원하시던 사진이 나오길 기대하지는 마십시오.

턱선 상담은 실제로 어떻게 진행되나

주사보다 상담이 더 깁니다. 무표정일 때와 악물 때, 정면과 옆선에서 얼굴을 보고, 턱에 손을 얹습니다. 사진에서는 같아 보이는 세 가지 원인이 손으로는 전혀 다르게 느껴지기 때문입니다.

그다음 어느 쪽인지, 어떤 비중인지, 각각에 무엇이 필요한지 말씀드립니다. 그 대화가 기대하고 오신 것보다 적게 하거나 아무것도 하지 않는 것으로 끝나기도 합니다. 그건 실패한 판매가 아니라 상담의 정당한 결론입니다.

한 번의 여행에 여러 시술을 묶는다면

순서가 중요합니다. 근육 이완은 몇 주에 걸쳐 보이고 리프팅은 더 일찍 보이므로, 순서를 잘못 잡으면 무엇이 무엇을 했는지 구분하기 어렵습니다. 짧게 계신다면 여행 일정을 먼저 알려 주십시오. 패키지가 아니라 그 일정에 맞춰 계획합니다.

솔직한 한계

유지하는 시술이고 영구적이지 않으며, 근육의 크기와 힘이 달라 결과도 다릅니다. 임신·수유 중이거나 신경근 질환이 있는 경우에는 적합하지 않습니다. 계획 전에 병력과 복용 약을 말씀해 주십시오.

이갈이나 악무는 습관이 원인이라면 습관을 그대로 둘 때 근육이 더 빨리 돌아옵니다. 간격이 예상보다 짧은 이유를 나중에 궁금해하시는 것보다 첫 방문에 말씀드리는 편이 낫습니다.
