한국 얼굴 필러는 누구에게 맞고, 언제 맞지 않는 도구일까?
필러는 볼륨을 더합니다. 눈밑 꺼짐, 납작한 관자놀이, 얇은 입술, 낮은 콧대, 깊어진 팔자주름처럼 무언가가 부족할 때 맞습니다. 문제가 '과잉'일 때는 잘 맞지 않습니다.

- 부위 수. 눈밑만 하는 것과 눈밑·관자놀이·입술을 함께 하는 것은 다른 계획입니다.
- 부위별로 필요한 제품 양. 깊게 꺼진 곳은 잔주름보다 더 많이 필요합니다.
- 제품 종류. 히알루론산 필러는 단단함이 서로 다르고, 입술에 맞는 제품이 코에 맞는 제품은 아닙니다.
- 속속을 계획에 넣는지, 그리고 몇 회인지.
- 한 번에 다 할지, 두 번의 방문으로 나눌지.

필러 후기에는 반복되는 주장이 몇 가지 있습니다. 의학적으로 맞는 것도, 아닌 것도 있습니다.
“당일 저녁에 바로 외출해도 된다.” 한 부위에 소량이라면 대개 맞습니다. 다만 바늘 자국과 가벼운 붓기가 남을 수 있고, 입술이나 눈밑은 그날 밤 사진 찍기에는 이를 수 있습니다.
“멍이 들면 시술자가 실수한 것이다.” 사실이 아닙니다. 미국 FDA는 멍, 붉은기, 붓기, 통증을 피부 필러의 흔한 부작용으로 들고 있습니다. 항응고 약이나 일부 영양제는 멍을 잘 들게 할 수 있으니 상담 때 말씀해 주시고, 처방받은 약은 처방한 의사에게 묻지 않고 끊지 마세요.
“첫 주의 멍울은 영구적이다.” 대개 아닙니다. 초기의 단단함은 붓기와 제품이 자리 잡는 과정인 경우가 많고, 상당 부분 첫 2주 정도에 걸쳐 부드러워집니다. 그 뒤에도 남아 있거나, 아프고 붉고 뜨거운 멍울은 진찰을 받아야 합니다.

누가 시술하고, 주사기에 무엇이 들어가는가

필러 후 서울에 며칠 머물러야 할까?

시술 당일. 마취와 주사 후 붓기, 붉은기, 작은 바늘 자국이 생길 수 있습니다. 대부분의 의사는 첫날 정도는 음주, 사우나, 격한 운동, 시술 부위를 세게 누르는 것을 피하고, 머리를 약간 높여 자라고 권합니다.
1–3일째. 붓기는 보통 첫 48시간에 가장 심하고, 입술은 이때 가장 도톰해 보입니다. 멍이 있다면 옅어지기 전에 더 짙어질 수 있습니다. 이 기간에는 사진 촬영보다 박물관이나 산책을 계획하세요.
7일째. 많은 분이 일주일쯤이면 붓기와 멍이 대부분 가라앉지만, 눈밑 멍은 더 오래갈 수 있습니다. 모양은 제품이 자리 잡는 2주 무렵에 판단하기가 더 쉽습니다.
출국 전날은 피하세요.
마지막 날 오후에 시술하면 붓기가 가장 심할 때 비행기를 타고, 멍은 아직 진행 중이며, 서울에서 걱정되는 부분을 확인받을 시간이 없습니다. 필러 후 비행 자체는 대개 괜찮고, 문제는 처음 며칠 동안 병원에서 멀리 있다는 점입니다.

알아 두어야 할 위험과 부작용
주사 부위의 붓기, 멍, 붉은기, 압통, 가벼운 울퉁불퉁함이 흔한 반응이며, 보통 며칠에서 일주일 정도면 나아집니다. 회복과 결과는 사람마다 다릅니다.
일반적인 의학 기준으로, 다음에 해당하면 대부분의 의사는 필러를 미룹니다.
- 시술할 부위에 감염, 입술 헤르페스, 여드름 염증이 있는 경우
- 같은 부위에 최근 레이저나 필링을 받은 경우
- 여드름약 이소트레티노인을 현재 또는 최근에 복용한 경우
- 임신 중이거나 수유 중인 경우
- 일부 필러에 섞인 국소마취제를 포함해 성분에 알레르기가 있는 경우
드물지만 심각한 합병증은 필러가 혈관으로 들어가는 것으로, FDA는 이것이 피부를 손상시키고 매우 드물게 시력에 영향을 줄 수 있다고 경고합니다. 피부가 하얗게 질리거나 얼룩덜룩하거나 거무스름해지는 경우, 통증이 계속 심해지는 경우, 시야에 어떤 변화라도 있는 경우에는 메시지 답을 기다리지 말고 바로 응급실로 가세요. 귀국 후 첫 주가 지나도 멍울이나 비대칭 붓기 같은 가벼운 걱정이 있다면 자연광에서 사진을 찍어 WhatsApp으로 보내고, 나빠지면 현지 의사를 찾으세요. 미국 CDC는 의료 여행자에게 진료 기록 사본을 챙기고 귀국 후 추적 진료를 마련해 두라고 권하니, 떠나기 전에 부위별로 사용한 제품 이름을 물어 두세요.
