요즘 한국식 입술 필러, 정확히 무엇이 다른가요?
제품 자체가 새로운 것은 아닙니다. 한국에서든 다른 나라에서든 입술 필러는 대부분 히알루론산입니다. 미국성형외과학회가 설명하듯 수분을 머금는 젤이고 몸에서 서서히 분해됩니다. 달라진 것은 어디에, 얼마나 넣느냐입니다.
입술 전체를 고르게 채우는 대신, 요즘 방식은 입술을 몇 개의 작은 구역으로 나눠서 봅니다.
- 입술 라인. 입술 가장자리를 따라 얇게 넣으면 볼륨을 크게 늘리지 않고도 윤곽과 큐피드 활 모양이 또렷해집니다.
- 가운데 볼륨. 윗입술과 아랫입술 가운데에 소량을 넣으면 주로 옆에서 보이는 부드러운 도톰함이 생깁니다.
- 입꼬리. 처진 입꼬리는 무표정일 때 피곤하거나 언짢아 보이게 하는데, 입꼬리 필러로 몇 밀리미터 들어 올립니다.
- 입술 몸통. 얇고 고르게 펴 넣으면 한국식 입술 하면 떠올리는 매끈한 표면이 됩니다.

입 주변에 활동성 입술 헤르페스나 감염이 있는 경우, 임신 중이거나 수유 중인 경우, 출혈 질환이나 심한 알레르기 반응 이력이 있는 경우라면 이번 여행에서는 입술에 주사하지 않겠습니다. 이미 입술에 필러가 있고 울퉁불퉁한데 진찰을 받아 본 적이 없다면 그때도 기다리고 싶습니다. 원인을 모르는 덩어리 위에 더 넣는 것은 순서가 틀렸습니다.

네, 도움이 됩니다. 참고 사진은 방향을 보여 주는 데 좋습니다. 더 또렷하게, 더 도톰하게, 입꼬리를 올리게, 혹은 그저 덜 납작하게. 다만 설계도로 쓰기에는 약합니다. 입술은 각자의 치아, 턱, 인중 위에 놓여 있고, 같은 양의 필러라도 윗입술이 짧은 사람과 긴 사람에게서 다르게 보이기 때문입니다.
대부분의 시술자는 아랫입술이 윗입술보다 조금 더 도톰하도록, 그리고 옆모습에서 윗입술이 아랫입술보다 앞으로 나오지 않도록 맞춥니다. 마음에 드는 사진이 내 얼굴에서 그 비율을 깨뜨린다면, 붓기가 빠진 뒤가 아니라 상담 때 말씀드리고 싶습니다.

시술 당일에 일어나는 일

떠나기 전에 제품 이름과 사용한 양을 물어서 여행 서류와 함께 보관하세요. 나중에 집 근처 의사가 입술을 진찰하게 되면, 가장 먼저 알고 싶어 할 두 가지입니다.
'잘못됐다'는 건 보통 무엇이고, 얼마나 흔한가요?
첫 주에 사람들이 입술 필러 부작용이라고 부르는 것들은 대부분 예상되는 반응입니다. 붓기, 멍, 압통, 붉어짐, 주사 자리의 작은 볼록함 같은 것들이죠. 미국 식품의약국(FDA)은 멍, 붉어짐, 붓기, 통증, 압통, 가려움, 발진을 피부 필러의 흔한 부작용으로 꼽습니다. 입술 멍은 보통 며칠에서 1~2주에 걸쳐 옅어집니다.
덜 흔한 문제는 의사가 봐야 합니다. 초기의 단단함은 붓기가 빠지면서 가라앉는 경우가 많지만, 몇 주 뒤에도 남아 있는 덩어리나 결절, 혹은 붉고 아파지는 덩어리는 집에서 마사지하기보다 진찰을 받아야 합니다. 히알루론산에 대한 진짜 알레르기는 흔하지 않지만, 필러나 많은 입술 필러에 섞인 국소 마취제에 반응하는 사람이 있습니다. 처음 이틀이 지나도 가려움, 두드러기, 붓기가 계속 심해진다면 연락해야 할 이유입니다. 입 주변의 주사는 입술 헤르페스가 잘 생기는 사람에게서 재발을 일으킬 수 있으니 상담 때 꼭 말씀해 주세요.
드물지만 심각한 것은 혈관 폐색입니다. 필러가 혈관 안으로 들어가거나 혈관을 눌러 혈류를 막는 경우입니다. 입술에는 표면 가까이 지나는 동맥이 있어서 이 부위에서는 주입 깊이가 특히 중요합니다. 경고 신호는 피부가 하얗게 또는 얼룩덜룩한 푸른 회색으로 변하는 것, 시술에 비해 지나치게 심한 통증, 그리고 나중에 생기는 물집입니다. FDA는 혈관에 의도치 않게 주입되면 피부가 손상될 수 있고, 드물게 시력 문제나 뇌졸중이 생길 수 있다고 경고합니다. 응급 상황으로 다루며, 히알루론산 필러는 히알루로니다제라는 효소로 녹일 수 있습니다. 입술에 히알루론산이 일반적으로 선호되는 이유 중 하나입니다.
