I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul and a board member of the International Petit Medical Society, and most of my clinical day is spent on injectables. When patients ask about Botox around the eyes, they usually attach a smiling selfie and ask: "What will this look like after Botox β and will I still look like me when I smile?" My answer, first: Botox softens the crow's feet that fan out when you smile. It does not fill hollows or remove under-eye bags, and a good result still lets your smile reach your eyes.
What does Botox before and after for the eyes actually show?
Softer, shallower crow's feet when you smile β with the smile itself intact. Crow's feet are expression lines made by the muscle that circles the eye. Botox relaxes the outer part of that muscle, so the lines fan out less. In an honest before and after, the "before" is a full smile with deep lines at the outer corners, and the "after" is the same smile with those lines softened.
Lines that are already etched in when your face is at rest improve less. Botox reduces movement; it does not rebuild skin. Where fine static lines remain, skin quality treatments can help more than extra units.
When do you see the result, and how long does it last?
Most people notice the effect within 3β7 days, with the full result around 2 weeks. That is when a fair "after" photo should be taken β the same smile, the same light, the same angle as the "before".
Results generally last about 3β4 months and then gradually fade. Duration varies by individual, dose and area. Regular treatment can help limit the deepening of repetitive expression lines, but it is maintenance, not a permanent change.
Which areas around the eyes does HERA treat with Botox?
Crow's feet β the outer corners of the eyes. At HERA, facial Botox is used for forehead lines, frown lines between the brows, crow's feet and jaw slimming, as well as the shoulder line and underarm sweating.
We do not inject Botox under the eyes. If your concern is a hollow under the eye, a shadow or dark circles caused by a hollow tear trough, the right tool is usually filler placed carefully with a cannula β see our eye rejuvenation filler page β or, for thin crepey skin, a skin booster such as Rejuran. I would rather send you to the right treatment than inject an area that will not respond.
What can go wrong with eye-area Botox?
Most issues come from dose and placement, not the product. Too much, or placed too close to the cheek, can make a smile look flat or unnatural. Tiny needle marks or slight redness may appear briefly, and a small bruise is possible because the skin around the eye is thin.
After treatment, avoid rubbing the area, lying down, and heat such as a sauna or intense exercise for a few hours. Downtime is otherwise minimal, and most people return to daily activities right away. A treatment takes about 5 to 10 minutes.
Which toxin does HERA use?
We offer Allergan Botox, Xeomin, Coretox and Newlux, and choose with you. Allergan is the original botulinum toxin type A with one of the longest clinical histories. Xeomin and Coretox are often chosen by frequent toxin users who want to minimise the risk of resistance. The clinical detail is on our Allergan Botox page.
For most crow's feet, the injector's dosing and placement matter more than the brand. I start conservatively; you can always add at the 2-week review, but you cannot take Botox away.
How should you read Botox before and after photos of the eyes?
Compare the same smile, in the same light, about two weeks apart. A photo of a relaxed face after treatment next to a big smile before treatment is not a fair comparison β crow's feet are dynamic lines, so they only show when you smile.
