Microneedling with Exosomes: Why the Two Are Combined

I'm Dr. Jong Seon Kim, a plastic surgeon at Gangnam HERA in Seoul. Patients arrive asking for "exosomes" the way they used to ask for vitamin drips — as a product rather than a procedure. The question I get is always the same: "Does it actually do anything?" The answer depends almost entirely on how it is delivered, which is why we combine it with RF microneedling.
Why combine microneedling with exosomes at all?
Because exosomes applied to intact skin mostly stay on the surface. The outer layer is a barrier — that is its job. RF microneedling opens micro-channels through it with ultra-fine needles while delivering radiofrequency energy into the dermis. The exosome booster is then absorbed through those channels rather than sitting on top of them.

Who it suits — and who it doesn't
It suits skin that breaks out, flushes easily, or has lost its barrier. It is not a lifting treatment and it will not tighten sagging. If your concern is laxity rather than texture, I will point you to lifting instead — using a texture treatment to chase laxity wastes your trip and your money.
Recovery and what to expect
The session takes about 30 minutes. Mild redness, warmth or slight swelling appears immediately and typically settles within a day or two. Sun protection afterwards is not optional — freshly needled skin pigments easily.
Results build across a series rather than from one visit, so if you are here for a short trip I will tell you honestly what a single session can and cannot achieve.
How it differs from microneedling alone
Plain microneedling creates controlled injury and lets the skin repair itself. That works, but the repair draws only on what your skin already has. Adding radiofrequency puts energy into the dermis rather than just the surface, and adding a booster gives the repair extra material to work with. The three together are why we run it as one treatment rather than a needling session with a serum applied afterwards.
If your skin is healthy and your concern is mild texture, plain needling may genuinely be enough — and I will say so rather than sell you the larger package.
How many sessions, and how to space them
Skin remodels on its own clock. Sessions are spaced to let that happen rather than to fill a calendar, and the change you notice at the end of a series is usually larger than the change after any single visit. Patients who photograph themselves weekly see it; patients who look in the mirror daily rarely do.
If you are visiting Seoul for a week, we plan what one or two sessions can realistically start, and what you would continue at home or on a return trip. I will not pretend a single session does what a series does.
What patients ask me next
- "Does it hurt?" Numbing cream is applied first and most patients describe pressure and heat rather than pain.
- "Can I wear makeup after?" Give the channels time to close — I will give you a specific window rather than a general rule, because it depends on the depth we used.
- "Is it safe for darker or sensitive skin?" Settings change with skin type, and this is precisely the kind of decision that should be a physician's rather than a fixed protocol.
The honest limits
Exosome products are not all the same, and the evidence base is younger than the marketing suggests. What I can tell you plainly is what we use, why the delivery method matters, and that the treatment is repeated rather than one-and-done. If someone promises you a transformation from a single session, be careful.
