Dermal Fillers for Sunken Eyes: Who They Suit and Where They Stop

I'm Dr. Kim Jong Seon, the filler specialist at Gangnam HERA in Seoul, and a board member of both the International Petit Medical Society and the Korean Society of Aesthetic Surgery (KSAS). The message I receive about the eyes is usually a close-up photograph and one question: "My eyes look sunken and tired β can dermal fillers fix that without surgery?" My short answer is often yes, when the hollow comes from lost volume above or around the eye. It is no when the real problem is a drooping lid or loose skin. And the amount that works here is smaller than most people expect.
What can dermal fillers do for sunken eyes?
They restore gentle volume to a hollow, tired-looking eye area, so the eye looks more rested without an incision. At HERA, sunken eyes are treated as part of our eye rejuvenation filler, using high-purity hyaluronic-acid filler that is KFDA- and FDA-approved. The change is visible the same day.
When the area above the eye has hollowed β the groove between the brow bone and the eyelid that makes the eye look set back β a small amount of filler softens that step, and the face reads as less drained. That is the goal: a livelier, refreshed look, not a filled one.
What filler does not do is lift. It cannot raise a heavy lid, tighten crepey skin or change the shape of the eye itself. I say that first because it decides whether filler is worth your money at all.

Are my eyes sunken, or is the lid sagging?
Sunken eyes are a volume problem; a sagging lid is a support problem β and only the first one is a filler problem. The two can look similar in photographs, which is why I examine you before agreeing to a plan.
- Hollowing: a visible dip above the eye, a brow bone that looks more prominent than it used to, and an eye that seems to sit deeper. Filler suits this.
- Loose skin: extra skin folds down over the lid. Adding volume underneath does not remove it.
- A lid that sits low over the eye: this is about how the eyelid opens, and it is assessed differently.
- Irregularities after double-eyelid surgery: filler can sometimes smooth an uneven area instead of revision surgery, which we judge case by case.
Many people have more than one of these. If hollowing is part of the picture, filler can help with that part. If it is not, I would rather tell you at the photo stage than sell you a syringe.
How is a sunken upper eyelid different from an under-eye hollow?
They are different areas with different plans, even though both get called "filler around the eyes". A sunken upper eyelid sits above the eye, under the brow. An under-eye hollow β the tear trough β sits below the eye and casts the shadow people call dark circles.
Many patients have both, and treating one can make the other more noticeable by contrast. That is why I look at the whole eye area together, and often the forehead and temples too, since a hollow upper face can make the eyes look deeper than they are. How is filler around the eyes placed safely?
With a fine blunt-tip micro-cannula, small amounts, and a doctor who maps the area before injecting. The skin around the eyes is the thinnest on the face, so filler placed too shallow or in too large an amount can look lumpy, uneven or slightly bluish. The area also carries important blood vessels, which is why depth and technique matter more here than the name on the syringe.
- Consultation and eye analysis: skin thickness, how deep the hollow is and what is causing it
- Design and marking: injection points are mapped so the result stays smooth and even
- Anaesthesia: topical numbing cream, with local anaesthesia if needed
- Cannula injection: high-purity HA placed at the correct depth with a blunt-tip micro-cannula to limit bruising
- Shaping and symmetry check: the doctor molds the filler and confirms both sides look balanced
The treatment is usually finished within about 30 minutes. Pain that keeps getting worse, skin turning pale or dusky, or any change in your vision after eye-area filler is not normal β contact the clinic the same day. These events are uncommon, and they are the reason this area belongs with a physician who can act at once.

What happens after sunken eye filler?
Most people go back to their day straight away, with a small bruise or mild swelling possible for a few days. Because the skin is thin, a bruise is more likely here than elsewhere on the face. It is easily covered with concealer and usually settles within about a week. Before you leave, you receive cold-care instructions and the precautions for the first days.
Hyaluronic-acid filler generally lasts around 6 to 12 months for domestic products and 12 to 18 months for imported ones, depending on the product and your metabolism. Results vary from person to person. The change fades gradually rather than overnight, and because hyaluronic acid can be dissolved, an adjustment is possible if one is ever needed.
What do patients ask me next?
Mostly whether it will look puffy, whether it hurts, what it costs, and whether a different treatment would work better.
"Will it make my eyes look puffy?"
Not if the amount follows your anatomy. Puffiness around the eyes almost always means too much filler, or filler in the wrong layer. I plan a conservative first session and review it, rather than finishing everything in one visit.
"Does it hurt?"
Numbing cream and the cannula technique keep it comfortable. Most patients describe pressure rather than pain.
"What will it cost?"

How should I plan it around a trip to Seoul?
Book it early in your stay, not the day before a flight or an event. A small bruise can take up to about a week to fade, and the shape is easiest to judge once swelling has settled. A review visit a few days after treatment lets me make a small adjustment if one side needs it.

Send a photograph of your eyes in natural light, from the front and at a slight angle, with your eyes open and looking straight ahead. Include an older photograph if you have one; seeing how your eyes looked a few years ago tells me a lot about whether volume has been lost.

What are the honest limits?
Filler for sunken eyes is temporary, works only on hollowing, and in this area less really is more. It does not remove excess skin, lift a heavy lid, erase pigment or replace eyelid surgery. What I can offer is an honest assessment of why your eyes look sunken, a conservative amount placed with a cannula, and a review rather than one large session.
