Forehead Botox for the First Time β A Real Patient Review

At Gangnam HERA, we see many patients who are trying Botox for the very first time, and forehead lines are one of the most common reasons. Imane (@imane.998) was one of them. In her caption she wrote that she "tried forehead Botox for the first time," and that her main goal was "just to soften those fine lines while keeping natural movement." She summed up the result as "smooth, refreshed, and still so natural." We want to use her case to explain how we plan a first forehead Botox treatment, what the appointment involves, when results appear, and why a natural result depends more on dosing and placement than on the product name. We will stay within what Imane shared publicly.
Why do forehead lines form, and how does Botox soften them?
Forehead lines are expression lines. Every time you raise your eyebrows, the forehead muscle contracts and folds the skin above it; over the years, those folds start to stay visible even at rest. Botox works by temporarily relaxing the muscles responsible for facial expressions, so the skin above them is folded less often and the lines soften. This is why Botox suits dynamic lines, the ones that come from movement, so well. It is also why Imane's goal was exactly the right one for a first treatment: soften the lines, keep the movement. A forehead that still moves a little looks like your own face; a forehead that cannot move at all is what people mean by a frozen look.
How do we plan a first forehead Botox treatment?
We start with a consultation to assess your muscles, concerns, and suitability. We ask you to raise your eyebrows, frown and relax, because where the lines form, and how strong the muscle is, varies from person to person. Because the forehead sits directly above the brows, injection points are planned with your brow position and expressions in mind. For a first-timer we usually take a conservative approach, using a careful dose placed to soften the lines while preserving natural movement. It is easier to add a little at a follow-up than to wait for an overly strong result to fade. We also discuss which toxin to use. HERA offers several brands, including Allergan Botox, Xeomin, Coretox and Newlux; Imane's caption does not name a brand, so we will not either.
What happens during the appointment?
The treatment itself is short, usually about 5 to 10 minutes. After the consultation, the area is cleansed and the injection points are marked precisely. The toxin is then injected with a fine needle, and the treated area is cooled and soothed. Discomfort is usually minimal because very fine needles are used, and numbing cream or cooling can be applied for sensitive patients. Downtime is minimal and most people return to daily activities right away. Tiny needle marks or slight redness may appear briefly. For the next few hours we ask you to avoid rubbing the area, lying down, and heat such as a sauna or intense exercise.
When do forehead Botox results appear, and how long do they last?
Botox does not work the moment it is injected. Most people notice the muscle-relaxing effect within 3 to 7 days, with the full result typically visible around 2 weeks after treatment. That two-week point is when we judge the result with you, and when a small adjustment can be considered if one area is still more active than the rest. Results generally last about 3 to 4 months and then gradually fade, with repeat treatment usually every 3 to 4 months to maintain results. Regular treatment can also help limit the deepening of repetitive expression lines, which is why some patients start early, as Imane's #preventativebotox hashtag suggests. Results vary from person to person, depending on dose, the area treated and individual metabolism.
What did this treatment not do, and what else might you consider?
Forehead Botox relaxes the muscle; it does not add volume and does not resurface the skin. If a forehead line is deeply etched even when the face is completely still, Botox alone may soften it but not remove it, and other options may be discussed at consultation. For a first treatment we also do not treat every area at once. Imane's goal was the forehead, so the plan was the forehead; frown lines between the brows or crow's feet can be treated in the same way if they bother you, but only when you ask for them. Who is forehead Botox right for?
Forehead Botox suits people whose horizontal lines show when they raise their brows or are starting to stay visible at rest, and who want a softer forehead without losing expression. It is a common first treatment for people new to injectables, because it is quick, the effect builds gradually and it fades over months. It is not suitable during pregnancy or breastfeeding, or for people with neuromuscular disorders, and we ask every patient to share their medical history and medications.
