필러 전문가란 피부 아래 해부학을 충분히 알아서, 어떤 주사는 하지 않겠다고 말할 수 있는 의사입니다. 한국에서 필러는 의료 행위이지만 의사마다 수련 정도가 크게 다르기 때문에, 중요한 질문은 나라나 브랜드가 아니라 주사기를 든 사람에 관한 것입니다.

사람들은 흔히 덜 중요한 신호부터 찾아봅니다. 아래 표는 그런 신호와, 실력에 대해 더 많은 것을 말해 주는 신호를 나란히 놓은 것입니다.

한국어로 전문 자격을 갖춘 의사는 '전문의'라고 합니다. '성형외과 전문의'나 '피부과 전문의'로 표기된 의사는 전문 수련과 전문의 시험을 마친 사람입니다. 통역사에게 부탁하면 질문 한 번으로 확인할 수 있습니다.

필러가 맞는 사람, 기다려야 할 사람, 대안

히알루론산 필러는 특정 부위의 꺼진 볼륨을 채우거나 모양을 다듬고 싶고, 제품이 서서히 분해된다는 점을 받아들이는 분에게 맞습니다. 임신 중이거나 수유 중일 때, 시술 부위 근처에 피부 감염이나 입술 물집(헤르페스)이 있을 때, 또는 주된 문제가 볼륨 감소보다 피부 처짐일 때는 대개 기다리는 편이 낫습니다.

자국에서 '어렵다'는 말을 들은 분들에게 자주 나오는 상황은 세 가지입니다.

  • 꺼짐이 아니라 눈 밑 지방 돌출인 경우. 앞으로 튀어나온 지방 위에 필러를 더하면 더 무거워 보일 수 있습니다. 꺼진 눈물고랑은 다른 문제이고, 진찰로 둘을 구분해야 합니다. 일반적인 신호 하나: 지방 돌출은 위를 볼 때 더 튀어나오는 경향이 있고, 눈물고랑 꺼짐은 눈확 뼈 가장자리를 따라 홈으로 남습니다.
  • 코 수술을 받은 적이 있는 경우. 흉터 조직이 혈관 주행을 바꿀 수 있어서, 코 성형 후 코 필러는 더 신중해야 하며 때로는 하지 않는 것이 답입니다.
  • 예전 필러로 입술에 멍울이 있는 경우. 위에 더 넣는다고 멍울이 해결되는 경우는 드뭅니다. 기존 제품을 먼저 평가해야 하며, 히알루론산 필러는 새로 넣기 전에 효소로 녹일 수 있는 경우가 많습니다.

필러가 맞지 않을 때는 다른 방법이 더 맞을 수 있습니다. 턱선을 따라 처진 피부는 실리프팅 같은 리프팅 방식에 더 잘 반응할 수 있고, 칙칙하거나 고르지 않은 피부결은 볼륨보다 스킨부스터(예: 리쥬란, 쥬베룩)의 문제인 경우가 많습니다. 어느 것이 맞는지, 혹은 아무것도 필요 없는지는 대면 상담에서 정합니다. 가슴이나 엉덩이에 대용량 필러를 넣는 것은 아예 찾지 말아야 할 시술입니다. FDA는 가슴 확대나 바디 컨투어링에 주사용 필러를 쓰지 말라고 경고합니다.

브랜드보다 중요한 필러 위험은 무엇일까요?

가장 심각한 필러 합병증은 제품이 어느 상자에서 나왔는지가 아니라 어디에 들어가는지에 달려 있습니다. 미국 식품의약국(FDA)은 필러의 위험으로 혈관 내 의도치 않은 주입을 꼽습니다. 혈류를 막아 피부 손상으로 이어질 수 있고, 드물게는 시력 문제나 실명으로 이어질 수 있습니다.

사람들이 가장 많이 묻는 세 부위에는 각각의 우려가 있습니다. 코 주변과 미간에는 눈 쪽으로 이어지는 작은 혈관들이 있습니다.

눈 밑은 얼굴에서 피부가 가장 얇아서, 제품이 너무 얕게 들어가면 푸르스름한 그늘이나 도드라진 줄로 보일 수 있습니다. 입술에서는 입술 동맥이 볼륨을 넣는 자리 가까이 지나가기 때문에, '입술 필러 부작용' 이야기는 브랜드보다 깊이와 위치의 문제로 귀결되는 경우가 많습니다. 관자놀이와 이마에서도 필러를 넣는 층 안으로 동맥이 지나가므로, 같은 깊이의 질문이 적용됩니다.

흔하고 일시적인 반응으로는 붓기, 멍, 압통, 그리고 점차 부드러워지는 작은 단단한 부분이 있습니다. 덜 흔한 것으로는 감염, 오래 남는 멍울, 주입한 자리에서 제품이 이동하는 경우가 있습니다.

주의: 당일 의사의 진료가 필요한 신호

시술에 비해 과도한 통증, 하얗게 질리거나 거무스름하거나 얼룩덜룩해지는 피부, 필러 후 시력의 변화. 즉시 시술한 병원에 연락하고, 이미 귀국했다면 현지 응급실로 가세요.

아무것도 예약하기 전에, 메시지나 통역을 통해 해외에서도 확인할 수 있는 신호가 네 가지 있습니다.

  1. 전문의 자격. 의사가 성형외과 또는 피부과 '전문의'로 표기되어 있고, 소속 학회를 밝힙니다.
  2. 상담한 의사가 직접 주사. 지금 만나는 사람이 주사기를 들 사람인지 직접 물어보세요.
  3. 환자 앞에서 밀봉 상태로 개봉하는 허가 제품. 개봉 전에 포장을 보여 달라고 하세요.
  4. 동맥에 대한 분명한 답. 의사가 시술 부위에 혈관이 어디로 지나가는지, 혈관이 막히면 어떻게 대처하는지 말할 수 있습니다.

리뷰는 직원이 설명을 잘해 주었는지 같은 서비스 질문에는 도움이 됩니다. 하지만 피한 합병증은 리뷰에 나타나지 않기 때문에, 실력에 대해서는 알려 주는 것이 적습니다.

귀국 비행기 며칠 전에 필러를 맞아야 할까요?

여행 초반에 맞고, 출국 전날은 피하세요. 붓기와 멍은 보통 처음 이틀 동안 가장 눈에 띄고 일주일쯤에 걸쳐 가라앉습니다. 그 며칠이 있어야 떠나기 전에 이상해 보이는 부분을 대면으로 확인할 여유가 생깁니다.

첫 일주일은 대체로 이렇지만, 사람마다 다릅니다.

  • 당일: 붉음, 붓기, 작은 바늘 자국. 그날은 음주, 사우나, 격한 운동을 피하세요.
  • 3일째: 붓기가 빠지는 중입니다. 입술은 아직 최종 모양보다 도톰해 보일 수 있고, 피부 아래로 작은 단단한 부분이 만져질 수 있습니다.
  • 7일째: 붓기가 대부분 가라앉고 모양이 최종에 가까워집니다. 최종 모습은 보통 2주 무렵에 판단합니다.

출국 전 주의

장거리 비행 전날 필러를 예약하면, 무언가 이상해 보여도 대면으로 확인할 시간이 없습니다. 시술과 출국 사이에 며칠을 두세요.

귀국 후에는 사후 관리 안내와 제품 정보를 진료 기록과 함께 보관하세요. 현지 의사가 무엇을 어디에 썼는지 볼 수 있습니다.
