Wondering what decides the under eye fat repositioning Korea cost, and whether the bags under your eyes need surgery at all?
Read this before you price out a trip to Seoul.
By the end you'll know what moves the cost, what reviews get right and wrong, how the methods differ and what recovery looks like on a short trip. That tells you whether to plan for surgery or for a smaller, non-surgical visit.
Hello, I'm Dr. Kim Jong Seon, Director of Gangnam HERA Plastic Surgery Clinic and a Full Member of the Korean Society of Oculoplastic Surgery.
At HERA, the specialist who consults with you is the one who treats you. International patients pay the same price as local patients, and an English interpreter joins the consultation.
I'm writing this because many people price fat repositioning when what they really have is a hollow, and many others book filler for a bulge that filler can't flatten.
Fat repositioning is lower eyelid surgery, and its cost depends mostly on the surgical approach, the anesthesia and how long you stay for stitches and checks. It isn't on HERA's treatment menu. What I offer is eye rejuvenation filler, which suits hollows rather than bulging fat.
If you can't tell which one you have, take two photos in daylight, one looking straight ahead and one looking up at the ceiling, and send them on WhatsApp with the question "bag or hollow?" Starting from your own photos keeps the first conversation about your eyes, not about averages.

Bags or hollows: which one do you have, and which method fits?
Under each lower lid sit three small pads of fat, held back by a thin membrane. With age that membrane usually loosens, so the fat pushes forward into a bag. At the same time the cheek just below tends to thin out, which leaves a groove, the tear trough, under the bag. Most tired-looking eyes are some mix of the two.
The methods people search for treat different parts of that picture:
- Fat repositioning is surgery, usually done through the inside of the lower lid. The surgeon releases the bulging fat and drapes it over the groove, so the bag and the hollow even out at once.
- Fat grafting takes fat from another part of the body, such as the thigh or belly, and injects it under the eye. It adds volume but doesn't deal with a bulge.
- Eye rejuvenation filler places hyaluronic acid into the groove to lift the shadow. It adds a small amount of volume and doesn't move any fat.
The American Society of Plastic Surgeons lists bags under the eyes and fatty deposits that show as puffiness among the concerns eyelid surgery treats. A quick mirror test helps you read your own face: hold a mirror at chin height and look up. Bags that swell more when you look up are usually fat pads. A groove that stays the same and looks darker under ceiling light is usually a hollow.
My reasoning is straightforward. If the bulge is the main problem, I wouldn't put filler on top of it. Adding volume under a bag that already pushes forward can make the area look puffier, and that is often where people start searching "under eye filler migration." In that case my advice is to see a surgeon who performs lower eyelid surgery. If the groove is the main problem and the bulge is small, filler can soften the shadow with a much shorter recovery.
| Fat repositioning | Eye rejuvenation filler | |
|---|---|---|
| What it treats | Bulging fat and the groove together | A hollow groove with little bulge |
| Anesthesia | Local with sedation, or general | Usually topical numbing |
| Recovery | Often one to two weeks of bruising | Usually a few days of swelling |
Say you're 56, you're flying in from Vancouver for ten days, and your bags puff up clearly when you look up. Filler would only add to the bulge, so I would tell you this trip is a surgery question, not a filler one. Now say you're 44, there's almost no bulge, and the groove casts a shadow that concealer can't hide by mid-afternoon. That is the face where eye rejuvenation filler makes sense, and where a short stay can work.
Two other things get confused with fat. Milia are tiny white keratin bumps that neither surgery nor filler treats; a doctor removes them separately. Thin, crepey skin over the area is a skin-quality problem, which is where a skin booster such as Rejuran sometimes comes into the conversation.
When you message, add one more photo: chin slightly down, under an overhead light, which shows how deep the groove really is. Put the dates you can be in Seoul in the same message, so the timing question is answered alongside the photos.

What decides the under eye fat repositioning cost for your eyes?
The under eye fat repositioning Korea cost isn't a single number, because the operation itself varies. These are the factors that usually move it:
- Approach. Working through the inside of the lid with no skin cut is one operation. Adding an incision under the lashes to remove loose skin is a bigger one.
- What's combined. A lid-tightening step, fat grafting or treating the upper lids at the same time all add operating time.
- Anesthesia. Local anesthesia with sedation and general anesthesia carry different facility and staffing needs.
- Days in Korea. Surgery usually means staying until stitches come out or the first check is done, so your hotel nights are part of the real total.
- Follow-up once home. Who sees you in your own city if something needs a second look.
If filler turns out to fit you better, the factors are smaller ones: how much product the groove needs, whether the hollow continues into the cheek and needs facial filler there too, and whether a second visit for a small top-up is planned. At HERA you pay the same price as a local patient, and you see a written estimate before anything is done. The under-eye filler price guide and the sunken-eye filler cost guide go into the filler side in more detail.
To get the figure for your own case, ask on WhatsApp what it would cost for your eyes, with the photos from the mirror test attached.
What do reviews get right and wrong about under-eye fat?
A few claims come up again and again in review threads and forums.
"Filler under the eyes always migrates." Partly true. Under-eye skin is the thinnest on the face, so too much product, or product placed too close to the surface, can show as puffiness or a bluish tint. A small amount placed deep in the right person is a different situation. The word "always" is the part that's wrong.
"Fat repositioning lasts forever." The repositioned fat generally stays where it was placed, so the change tends to last for years. Your skin and cheek keep ageing, though, so the area will still change over time.
"A Korean under-eye balm or concealer can shrink bags." Creams can hydrate the skin and concealer can hide a shadow, but no topical product moves a fat pad. If a balm review describes bags disappearing, that's camouflage or swelling going down.
"Fat grafting recovery is quick." Grafting usually swells more than filler, and how much of the grafted fat survives differs from person to person, so small lumps can appear. It's surgery, with surgical recovery.
One doctor, a sealed vial and a written estimate
The specialist who consults with you is the one who performs your treatment; it isn't handed off. Only genuine, approved products are used, and the sealed vial is opened in front of you. We don't rush through high volumes, and the plan covers only what you need.

What does recovery look like on a short trip?
For under-eye filler, general experience looks like this:
- Same day: some swelling and possibly a small bruise at the injection point. Most people go back to their hotel and carry on with a quiet evening.
- Day 3: swelling usually settles, though the area can look a little puffy in the morning. Small bruises can be covered with concealer.
- Day 7: most of the swelling has typically gone, and you see the shape the filler gives.
Fat repositioning surgery runs on a longer clock. Bruising and swelling commonly last one to two weeks, and an incision under the lashes means stitches that need removing, so most surgical plans keep you in Seoul longer than a filler visit would.
Before you fly
avoid having treatment the day before your flight home. The U.S. CDC's medical tourism guidance advises planning follow-up care before you travel and notes that flying soon after surgery can raise the risk of blood clots.
Risks and side effects to know
After under-eye filler, the common reactions are swelling, bruising, tenderness and small lumps, and they usually fade within days to a couple of weeks. Puffiness or a bluish tint can persist if product sits too superficially. The U.S. FDA lists a rare but serious risk: filler entering a blood vessel, which can damage skin or, very rarely, affect vision. Eyelid surgery has its own risks, including dry eye, asymmetry and the lower lid pulling down.
As general medicine, you should wait or choose another route if you have:
- an infection, rash or inflammation around the eyes
- a laser or peel on the area in recent weeks
- isotretinoin treatment now or recently
- pregnancy or breastfeeding
- an allergy to an ingredient, including salmon-derived PN if a skin booster is discussed
- a large fat bulge, where filler is the wrong tool
If something feels wrong after you fly home, sudden pain, skin turning pale or dusky, or any change in vision needs emergency care that day. For a lump or puffiness that doesn't settle, see a doctor locally and send a photo on WhatsApp so the treatment details are on record. How your eyes respond is individual, which is why the plan is made in person.
Visiting HERA in Gangnam
HERA is at 4F, 9 Seocho-daero 73-gil, Seocho-gu, Seoul, three minutes from Gangnam Station. Hours are Monday and Friday 10:00β20:00, Tuesday to Thursday 10:00β19:00 and Saturday 10:00β17:00, with lunch from 13:00 to 14:00. We're closed on Sundays and public holidays. HERA is registered with Korea's Ministry of Health and Welfare to receive international patients. If your hollow extends to the side of the face, the temple filler cost guide is worth a look before you plan.
Your first message can be short: one daylight photo looking up, your travel dates and the question "is this a bag or a hollow?" You can ask that before booking anything. When you come, I examine your lower lids in person and recommend what fits your eyes, including when a surgeon's operation would serve you better than filler.
FAQ
Frequently asked questions
Q1Does HERA perform under-eye fat repositioning surgery?
No. HERA's menu is non-surgical. For under-eye hollows I offer eye rejuvenation filler. If a bulging fat pad is the main problem, a surgeon who performs lower eyelid surgery is the right person to see.Q2Can filler replace fat repositioning?
Only when the problem is mainly a hollow groove with little bulge. Filler adds volume; it doesn't move fat. Placing it under a bag that already pushes forward can make the area look puffier.Q3Do international patients pay more at HERA?
No. International patients pay the same price as local patients, and you receive a written estimate before treatment. To get the figure for your case, ask on WhatsApp with a photo.Q4What is under-eye fat grafting, and how long is recovery?
Fat grafting moves fat from the thigh or belly and injects it under the eyes. It's a surgical procedure, usually with more swelling than filler, and grafted fat survival varies, so small lumps can occur. HERA doesn't offer it.Q5How do I get rid of milia under my eyes?
Milia are tiny keratin cysts, not fat. Filler and fat repositioning don't treat them; a doctor usually removes them with a small extraction. Avoid squeezing them yourself near the eye.
